Wellbrooke of Avon is a 70-bed corporate facility characterized by high registered nurse staffing levels, zero reliance on contract labor, and a 4/5 overall federal rating.
The data indicates that Wellbrooke of Avon maintains staffing levels and retention rates that exceed both state and national benchmarks. Records show a nursing staff turnover rate of 29.2%, which is considerably lower than the Indiana average of 47%. Furthermore, the facility reported zero use of agency or contract staff, a metric that often correlates with higher continuity of care. Registered nurse staffing of 1.29 hours per resident per day is notably higher than the state average of 0.66 hours. While weekday staffing is robust, data shows a 10.4% decrease in nursing hours during weekends, dropping from 4.06 to 3.64 hours per resident per day.
Inspection history reflects a downward trend in findings, with the most recent survey in March 2025 identifying five deficiencies, compared to eight the previous year. Recent documentation noted issues regarding the implementation of measurable care plans, pressure ulcer prevention, and physician oversight of progress notes. Despite these findings, the facility has not incurred any federal fines or payment denials in the current cycle. Quality measures for both long-stay and short-stay residents are rated at the highest level by federal regulators, and the facility has maintained stable leadership with no administrator departures reported in the last year.
Registered nurse care levels are 1.29 hours per resident per day, which is nearly double the Indiana state average of 0.66 hours.
The facility utilizes 0% contract or agency staffing, maintaining 100% permanent staff for all nursing and assistant roles.
Total nursing staff turnover is 29.2%, significantly lower than the state average of 47%, indicating higher staff retention.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"I was at Wellbrooke for almost 9 weeks after falling and fracturing a kneecap and shoulder. I received excellent care and have only good things to say about Wellbrooke. Thanks especially to the therapy team who is top notch. They all work well together and are very kind and encouraging in providing excellent care. I worked mostly with Robert, Chai, and Jaladhi and want to thank them for helping me reach my OT and PT goals. The rehab director Jodi is amazing and keeps everyone organized and on track. Her trainee Courtney was very personable and helpful. Kathy and Heidi in the Activities Department always encouraged residents to participate in fun activities. They are the best! Samantha took care of the insurance side and arranged my home therapy after discharge. In addition, the facility is clean and inviting and has staff who genuinely care for the residents. I can't mention everyone but thanks to all at Wellbrooke who helped me through a very difficult time."
"I live at Wellbrooke of Avon assisted living. The staff are friendly, helpful, and I am very happy. The whole building is kept clean. The therapy team is fantastic working with me and other residents. The food is great! I would recommend and do recommend this facility to anyone looking for somewhere to call home. I gladly call Wellbrooke of Avon my home."
"Wow what a great place! My Mom was recently at Hendricks Regional Hospital & they suggested she go to Rehab after she was released. We chose Wellbrooke of Avon from a list provided by the hospital. I have nothing but praise for this place!!! My sisters & I were scared …. the workers at Wellbrooke of Avon welcomed us with open arms. We were allowed to tour the facility & ask questions even before Mom moved in. Since Mom has been in Wellbrooke of Avon, her therapy has been amazing & she is getting much stronger. Whenever I have a question or a concern I immediately get an answer from someone that works there. Audrey at the front desk has been wonderful. I can stop at nurses station, there is always someone there and willing to help. The Social Worker, Samantha, is the BEST! She has walked us through so many issues. I have stopped by her office, talked to in the hallways & called her from home … she makes herself available & listens so well. Thank You Wellbrooke of Avon! Mom is leaving Rehab …. but we are keeping her there. I met Victoria in Assisted Living who is just as caring as the Rehab Employees. My sisters & I have agreed to move Mom into Wellbrooke of Avon Assisted Living."
"This place is amazing. My Aunt has been in Wellbrooke several times and every time she has been treated with amazing care. Not only do they treat there patients/residents wonderfully, they have treated us (her family) with the same amount of kindness and compassion. You will not be disappointed with Wellbrooke of Avon."
"The care I received was excellent and very personal. Most took time to get to know me which was comforting in a tough situation. Food is pretty good...not always warm enough. I would consider Wellbrooke again if the need arises."
"I was here 3 yrs ago with a broken hip. Stayed for 4 weeks. Everything was great then. This time I was here because of ear cancer with a lot of stitches on my neck, a wrist where surgeons took out an artery & vein & a skin graft on my thigh. Everyone here has been so nice & caring. The nurses, OT/PT people & dining people really take time to treat me with care & concern. I would greatly recommend this facility for anyone needing rehab."
"RUN! They look good on the surface level, but are really there just for the money. They don't have enough care staff to appropriately care for patients. Transportation is a nightmare, often appointments are gotten to excessively late risking missing the appointment slot or completely missed and rescheduled ignoring high importance appointments because they outsource 99% of their transportation and then they charge you additionally for that transport because their in house service is almost non-existent. The PT staff don't monitor patients very well while on the bike leading to pressure wounds and those wounds are not given enough attention. The care staff spill urine on the carpet emptying catheter bags and don't clean it up making the room smell and don't care until it bothers them. The additional meal supplements such as protein shakes that are supposed to be provided daily are not provided. They also don't communicate well at all, they don't want anything in writing so they can lie and cover their mistakes. The only positive is that some of the nursing staff are nice."
"My mother was transfered out of this facility yesterday due to the incompetence of therapy department which did nothing for my mother in new facility On day one therapy worked with my mother without a hoyer without not being forced to use the parrelle bars for rehab and she stood from the bed to a walker. So I would like you to explain why they accomplished something in one day that your therapy department could not do in almost 3 weeks."
"Wellbrooke of Avon has been such a pleasant surprise! I had such low expectations of nursing homes and when my mother was in need, we chose here after touring 8 different facilities! The staff are friendly and professional and all have called my mom by name since the very beginning. It is clean and does not smell like a nursing home. My mom raves about the food and enjoys all of the activities, especially bingo and when there is live music. My mom will move over to the assisted living side to participate in their Almost Home program to give her a few more weeks to rehab and build strength before going home. The staff communication regarding insurance has been timely and effective. All in all this has been a much better experience than we ever expected."
"I really appreciate the care my loved one receives at Wellbrooke. The Staff engage with the residents and take care of them as if they are their own family. I am so grateful!"
