    DO NOT SEND ANYBODY HERE. i went on thanksgiving and my mom with trigeminal neuralgia was complaining of pain, the dr tapped her leg and said “see. all better”, she asked for a doctor but they sent in a nurse instead that shoved meds down her throat. she was gagging and throwing up, saying “please no, please dont”. she forced fed her the meds AGAIN. they don’t let her make calls either. dont worry i’m going to handle this and let all of massachusetts know what these people are all about.

    My 1st time at a facility like this and my wife chose it for the cleanliness and PT equipment. I was on 3rd floor E and the staff, nurses and patient aids were very courteous, patient and helpful. Special words for my physical therapist "Shohel-PT", he took the time to explain every exercise and the reason being done. He was very supportive and became a major player in my recovery.

    DIVERSICARE OF BIG SPRINGS

    My grandmother fell at her home. She fractured her arm that required surgery. She also fractured her hip that need to heal in rehab. My grandmother was there less than a week. The facility left her unattended in a wheelchair and she fell again. This time she fractured her neck and lost movement in her right arm. The facility took no responsibility of their error in judgement nor showed any remorse. I have to remind the staff about her health needs and cleanliness on a regular basis. Medications were changed with consulting appropriate physicians or family members. Bandages are not changed regularly like required. The staff documents in patient's charts that things are being done when they are not. Management is poor. Some of the staff is unfriendly. Staff act unprofessional in the hallways. They talk loud and giggle in the halls while at work. The staff doesn't communicate with each other or the family members. If you need a rehab or nursing home for your loved one...DO NOT CHOOSE THIS ONE!!!

    PT and OT are amazing top notch. Work as a team, challenge you! Individually everyone one is very nice and caring. They do not work as a team. There is no internal communication and i have never been somewhere that the vibe is thats not my job. Nurses and aids barely speak to each other or ignore each other. Kitchen staff doesn’t communicate with the nurses and aids or the nurses and aids choose not to listen. The food is repetitive and not the healthiest. nothing changes. Grilled cheese and toasted cheese are not the same. Breakfast should not be carbs only. Toast goes stale when you put it under a lid. Cream chipped beef should be served in a bowl when you know it is going to sit a while before it gets to the patient and please please give them more than one tbsp. Hot dogs should not be served more than once a month and fries get soggy when you put them under a dome. Condiments should be real mustard and real ketchup. If necessary have multiple menus for the health of the patient. Just like everything else, one size does not fit all! Filling patients with carbs and all canned vegetables and fruit is not acceptable. For the money that patients and insurance companies are paying, i don’t think it is too much to offer some kind of fresh fruit or vegetable with each meal. Absolutely no variety! I feel bad for those who cannot order out or have family who can afford to bring outside food in. Wawa and fast food get old very fast. For the record, i am 55 years old with stage 4 cancer. I fell and fractured my hip which is what ultimately landed me here. Any of the changes i mentioned would vastly improve the facility and i didn't eve mention the treatment of patients with dementia or the horrible smell from upstairs. I want to say a special ty to the cleaning staff especially Tasha. They were always kind and took a moment to ask how you are and mean it. I want to say to Lizette, Tanya, Lizzette, Valerie, Iesha, Tasha, mikre, amber and the whole PT,OT team but especially Alicia. Thank you fir the care you did offer. I never would have gotten through this with out you all. There is more but i cant remember their names. Thank you for the care and i hope things change and fast.

    I have just moved my dear mother to West Orange Center for Nursing. I must say, having had my mom in other nursing homes, what sets this nursing home apart from the rest is the level of care and customer service. Someone is always available to help, including weekends. They seem to sincerely care about my mom and her well-being. Their activities staff is creative, caring, patient, and inclusive. My admissions representative, Fatima, has gone above and beyond to ensure my concerns are addressed, as does Jamie, the Nurse Manager, and Nadine, one of the lead nurses. Apologies for rambling; however, when it comes to parents, you want all the feedback you can get. So far so good.🙏🏾😌

    LEXINGTON PARK NURSING & POST ACUTE CENTER

    I recently had an illness which required rehabilitation. I can not say enough good things about Lexington Park Rehab! The staff was caring, attentive, and all around absolutely amazing. The Physical and Occupational Therapists worked with me daily to get my strength back. The food was great,,and plentiful servings. The atmosphere was clean and very welcoming. I met great patients there and they encourage socializing. If I could give it 10 stars I would. I highly recommend Lexington Park Rehab.

    KING DAVID NURSING AND REHABILITATION CENTER

    I visited several times to see my loved one. The entrance was inviting. Check in process was easy. The receptionist was friendly. Lots of daily activities. My loved one raves about her doctor, PT and OT. They are very supportive and encouraging. Her room was large and clean. It's really cold in the room. Linens are changed nearly every day and bed is made. Concerns about the dietary restrictions were addressed but not rectified. Food missing off her tray. Concerns about the toilet leaking urine on the floor have gone unaddressed for over a week according to my loved one. Some of the GNAs are sarcastic and doing the minimum. My loved one asked staff if they could have a sheet on the floor to soak up urine but was denied. Her footies get wet and when asked for a clean pair, was told she was issued one pair on admission this she would not receive a clean pair. This is concerning 😟. Otherwise, it's been a positive experience for my loved one who is looking to come home.

The receptionist Mrs. Elsa and whatever else position that she may hold there was very helpful, polite, friendly, and professional. She is true to her calling and career-oriented. Mrs. Elsa made us feel very welcome and she Was exceptionally happy and an amazing person. She is a Great asset to the company and I hope that she is well appreciated. The facility was very clean and fresh. I was very impressed. Thank you for your service and treating patients with tender loving care that they deserve. The staff demonstrated professionalism and passion. Mrs. Elsa is dedicated to her job, full of compassion, and love, and is committed to the cause of the patients that are being housed at West Orange Center.

    My mother has been at NHC Bristol for four months. From mom's first day of arrival, the care received has been above and beyond what I have experienced both as a daughter of an aging parent and a Geriatric Care Manager in healthcare and geriatrics. Upon mom's arrival, I met with the management team to discuss her care plan and my concerns as I live out of state. Immediately I was impressed with the number of staff that met with me and, most importantly, listened as I provided information crucial to her care, particularly her medical and mental health. As my mother has several complex chronic diagnoses, the quality of medical care and the way it is delivered is first and foremost to her family. Through moms' good days and bad days, the staff, including the Administrator, Director of Nursing, physicians, nurse practitioners, nurses, CNAs', dietary, therapy, laundry, housekeeping, and maintenance, have been accessible, invaluable, professional, and personable. Activities have played an essential aspect of socialization for mom as she is social but often needs to be encouraged to attend events. As a result, mom participates in more activities longer, primarily due to the activity department's patience and gentle persistence. In addition, the administrative staff has been essential in assisting me with obtaining insurance that best meets mom's needs as a resident and if she returns to the community. The team has always been accessible to me, day or night, for any and every concern or question I have. There has not been one time that I have called and not received a return call. For anyone who has experienced the anxiety and stress of caring for an aging parent, knowing that your parent is in a safe place is essential. A family needs to know that their loved one resides in a facility where quality care abounds, concerns are addressed, dignity is respected, and questions get answered. NHC Bristol excels in every area. I know that at NHC Bristol, mom is safe and taken care of, and with the progression of her illnesses, you are taking care of her better than we could do. Thank you for all you have done and continue to do, and most importantly, thank you for providing my family and me the peace of mind that mom is in great hands!!!

    Had a scheduled appointment with the dermatology department. Called a week before to get directions and instructions. I left a clear message, and call back number. My call was never returned. I called the morning of the appointment and left another message. This time I explained I needed to leave by a certain time and asked them again to please call me.Having never been to this office or hospital, I needed directions. I clearly told them this. No call back again. I had to leave by a certain time to travel from Herkimer County. We arrived at the hospital about 15 minutes before the appointment and it took us about 10 minutes to find a parking space. I have a disabled parking tag but there were no spots available and so I had to walk from the main lot. It took another 6 minutes to get into the building.and then it took about 8 minutes to find the dermatology office. So we arrived into the dermatology office 9 minutes after our appointment time. There was no one else waiting to be seen. The desk person told me that we would not be seen because we are late! We explained the situation but she said it is policy. While she said she is the one who called back messages she would not tell us why she failed to return our call each time. In addition she said she could not make a new appointment for us because the provider is leaving. I find this very unprofessional as well as uncaring and completely unfair.

    CHARLOTTE BAY REHAB AND CARE CENTER

    When I was sent here, I thought it was a rehab facility only to learn it's a nursing home. My original diagnosis was much worse. Fortunately, my case was not as severe as originally believed. I can gladly say the PT and OPT were exceptional and very encouraging. I rated the facility 2 stars due to the lack of care to patients. When I first arrived, I was unable to walk. I called for a assistance to use a bed pan. I waited 45 minutes and no one came. I managed to assist myself but spilled the bed pan when I removed it. This became my usual practice since nobody would come. I timed my roommate once and her light was on for 1 and 1/2 hours. She was told she had to give the staff time to assist her. I learned very quickly to work hard at PT and get out ASAP. The food is very basic. You get a menu, however, 90% of the time you get something entirely different. Too many carbs and the meals are not balanced. You get one very small pack of butter, NO salt or pepper, or no condiments at all. Sometimes nothing to drink. The facility needs a good cleaning and maintained. The staff always use gloves and are disposed prior to leaving your room.

    Above and beyond my expectations!! I was on the rapid rehab hall and I can honestly say that I was blown away with everything from the welcoming that I received when I arrived, to the staff’s thoughtful, kind, caring, and heartwarming regard they had for me from the moment I walked in the doors until the time I left!! I was treated like a first class citizen and the staff really went out of their way to accommodate to my needs. My light was always answered promptly, housekeeping made their presence known and did what was asked of them, the therapy staff was vigilant to how I was feeling and worked with my schedule to eliminate stress and discomfort during my sessions. I just can’t say enough about this place because I was truly surprised by the level and quality of care, which for me has been a very hard thing to find in these places.I’ve unfortunately had to frequent hospitals and rehabs more than a few times and never have I said I would return to any of them but, believe it or not, I would come back here hands down if I had to! ( god forbid) To the families trying to decide where to place their loved one, please take my advice and trust in this facility. I know everyone has a different of opinions and experiences but, this was a place that god sent me to heal and recover so all I can tell you is if the opportunity is presented to you, trust that it’ll be the right fit for your loved one. Thank you so much again to all the staff for the support, love, and care given to me during my stay, you blew me away with your kindness and definitely made an impression on me that’ll stay with me forever. Also, as a bonus, I made a few friends that I didn’t have before my visit here where, in most cases, I couldn’t wait to get away from the majority if not all workers at the other places! One thing I can say that needs some improvement is the food! Could absolutely use a little love in the kitchen! Well, I hope this helps make your decision a little easier as it’s not a situation you can take lightly. Good luck in your choice, and the recovery and future of your family member and god bless!

    I put my mom in Lake Bennett Rehab after she became so weak with a stay in the hospital with pneumonia. I had a client several years ago I took care of for 8 years in Lake Bennet. I chose it because of the excellent Rehab team. She progressed really well. But 2 days before she was to come back to our home, where she'd been for a year. I realized she would have better care, socialize with others. Plus, they have access to nurses and doctors. She has meals with others, talking and laughing, gets her nails painted. Lake Bennet has a wonderful staff, the CNA 's are so sweet to my mom. All the staff is very helpful. When she lived with us it was bed to chair and visits to the hospital. I know I made the right decision for mom's quality of life. Thank you to the staff at Lake Bennet

    My mother was in this facility. She woke up one morning for breakfast. She had a headache so she told the nurse that she was going to lay back down which was around 9am. When lunch came no one checked on her or even woke her up to take her beds. Long story short, not one nurse or aide try waking her up all day or night so she did without medications or food. The next day around 9am a nurse went into her room to wake her and my mother was unresponsive. My mother had a stroke the prior morning. She laid there for over 13 hours and no one checked on her. When we got to the hospital she was on life support, they told us there was nothing they can do for her. My mother was brain dead because they waited too long to call an ambulance. We had to take my Mom off of life support and she passed away. And not to mention, they weren't giving her the blood thinner that she was supposed to be taking.

    If I could give zero stars, I would. Absolutely horrible. My loved one was there recently and he contracted rashes and sores due to neglect. We had to practically force the employees to do the bare minimum (shower, change his clothes, walk him around). The place was filthier than the hospital and staff were hard to find. Everyone was clearly under cared for. It was difficult to get him out of the facility due to the employees’ lack of communication and effort. It took them days to get my loved one his medications which caused him more problems! I wouldn’t wish this place upon my worst enemy. If you care about the well being of your loved ones DO NOT take them here!

    From the very first day, this treatment center gave us something we desperately needed — hope. My daughter came in unable to even stand on her own. In just 3 weeks, because of their dedication, skill, and genuine care, she is walking with a walker. That progress didn’t happen by accident — it happened because of a team that truly goes above and beyond. Every therapist treated her like more than just a patient. They were patient, encouraging, and consistent. They pushed her when she needed it and celebrated every small victory right alongside us. During one of the hardest seasons of our lives, they became part of our support system. The knowledge, compassion, and commitment this team shows every single day changes lives. It changed ours. I will forever be grateful for the role they played in my daughter’s recovery. If you’re looking for a place that truly cares and produces real results, this is it.

    SHORE ACRES CARE CENTER AND REHAB

    Most disturbing facility I’ve ever seen. My mother paid for the “5 star insurance” for THIS? I took her out after 2 days & came to take care of her from out of state. Had her physician friends not dropped in to visit, She may have fallen since somehow they allowed her to wander from her room to the common rooms. I have no idea how she had the strength to do that without falling and busting her head open. Fall risk isn’t a thing there. This place should be illegal based on unsanitary conditions & understaffing alone! SOMEONE is getting paid for these beds & passing on the lack of care to the patients. She said the food was good so perhaps the kitchen manager is the one that cares, kudos to him/her! Look for another place & take care of your loved one until then. I absolutely broke down when I had to leave her there. Flew back to get her in 48 hours. No way!

    Review for Mr. Shohel Noorami – Downtown Brooklyn Nursing /Rehabilitation Center I cannot say enough wonderful things about Mr. Shohel Noorami, the exceptional physical therapist who worked with my mother during her stay at Downtown Brooklyn Nursing/Rehabilitation Center. From the very beginning, Mr. Noorami demonstrated incredible patience, care, and professionalism. Due to her illness, it was extremely difficult and painful for my mom to move her legs. It was a slow and often frustrating process, but Mr. Noorami never once showed anything but encouragement and understanding. His calm and reassuring demeanor made all the difference. He took the time to explain every step, worked at her pace, and celebrated each small victory with her. Thanks to his dedication and expert guidance, my mother is now walking again—a milestone we weren’t sure would be possible. She can't stop talking about him and often mentions how supported and motivated she felt during her sessions. We are beyond grateful for Mr. Noorami’s compassion, knowledge, and the exceptional care he provided. He truly went above and beyond, and we thank him from the bottom of our hearts.

All day seating around with their phone looking at social media. The first time I went to see a family member for 5 hrs. Not even once they came to checked on my family member.

    Lake Bennet Center for Rehabilitation and Healing has done a remarkable job caring for my grandfather. From the moment he arrived, the staff has been warm, friendly, and incredibly accommodating. The director, Shivone, has been an absolute delight to communicate with. She is attentive, compassionate, and truly invested in the wellbeing of each resident and their families. Erica and Pam have also been wonderful. They consistently follow up, provide updates, and make sure my grandfather’s needs are met with care and professionalism. The nursing staff has been nothing but friendly, supportive, and patient throughout his stay. It brings our family real peace of mind knowing he is in such capable and caring hands. I highly recommend Lake Bennet Center for Rehabilitation and Healing to anyone looking for a facility that treats residents with dignity, kindness, and genuine compassion.

    This facility is terrible ! It’s not only the staff. The administrators are the worst ! They are not professional and do not look out for the patient’s best interest. If you ask three different people the same question you will get three different answers. Thats IF you can even find someone in authority to talk to. Patients are left alone for extended periods of time even when they ask for help. My parent was staying there and was left covered with sheets and blankets that had urine and feces on them. My parent also had terrible bedsores from being left laying flat for several weeks and not being turned to the side. There is too many reasons to list without writing a book. Beware !!!!

    This place is awful and I believe fosters aa poor attitude related to patient treatment. My wife has been at this facility 3 times (Unfortunately I could not control her going to some other facility) She has been a patient at this lousy facility three times, Once for 6 days, once for 7 days, once for 36 days. Each time due to the negligence of this facility she had to be transferred to the ER for the same issues as they could not properly give her the correct treatment to ensure she did not end up in the ER. The most recent time she was transported she was admitted with acute respiratory failure, Influenza B, Pneumonia, High lactic acid, a UTI and several large pressure wounds for which I was never informed of and she developed during her last stay. Also, my wife had 1 personal item, her prescription glasses. When transported to the ER this facility could not even make sure her glasses went with her, Unbelievable. Recently, she experienced respiratory difficulties for about a half hour. I rang the nurse call. After 5 minutes when no nurse came, I went to the nurses desk to explain the concerns and asked for a nurse to please come down to her room. A NURSE DID NOT SHOW UP UNTIL 30 minutes later. How unprofessional. During my visits, I have experienced windows of up to 4 hours or more before a nurse stopped by. My wife is unable to speak and press a call button by herself, so getting frequent visits is very important. I find that the treatment at this facility is inadequate and personally a danger to patients. I truly don't believe this facility really cares about patient well-being. Fair to say we will never be going back to this facility and I am in process of submitting formal complaints with the proper regulatory organizations to ensure they are investigated.

    I have continuously talked to the staff including Ms. Alice concerning care at this facility. I had to start doing my mom’s laundry a couple of years ago because things would go missing, bleach all over the clothes, and stains left behind. I was asked if I wanted them to do her soiled pads and I said no. When I picked up her laundry yesterday the picture said it all when I went to do her laundry. They have more agency working than staff. Turn over is ridiculous. Some of the ones that are on staff their have really been good to my loved one, but some of the agency, they are just there and that is it. For instance this past Sunday, no CNA was scheduled, they ended up calling in an agency and a new hire. The agency person was working her butt off and also training the new hire for the company. Also, this past Sunday, one CNA to care for 20 patients. Quite a few of these patients are hand fed and have to be lifted by a hoyer lift which is required 2 staff members. The agency CNA's dont know the patients and a lot of them don’t read up on them which in turn can put a patient in danger. Everytime someone in charge gets something corrected they end up quitting. Promises keep getting made and never kept. I am sure this is not the only nursing home that is like this, but as of now this is my loved one’s home and I expect her to be taking care of. Other than that it is a clean place and does not have the smell that other homes have.

    This is my third stay at Morning View. There have been numerous changes and new construction since the ownership has changed. The nurses, therapists, and staff are wonderful. The Activities Director Paige and her assistant Gracie were fantastic. Each day things were planned that got the patients out of their rooms participating in games, popcorn/coke parties, gift exchanges, and Christmas in July activities. So being healed physically was paired with healing spiritually and emotionally in the Activities Program.

    When getting lab work done at the Lowville Medical building, you have to call hospital admissions first, which would be fine IF the admissions clerk didn’t act like either you are doing something wrong or they have no idea what you’re talking about. If this call is going to be required, then the clerks need to be trained that they will be getting calls from people getting their lab work done at the Lowville Medical building, so that they don’t seem completely baffled that someone is calling to register for lab work. I appreciate that Kerry is at the Lowville Medical building and able to provide this service at that location!! She does a great job there!
