    Place is amazing. My wife is here and I wouldn’t trust her anywhere else. Admin Jake is tremendous and goes above and beyond. Angela director of nursing is great too. Nurses are nice and friendly. Therapy is tremendous. Clean building. New rooms are nice. Housekeeping is nice and laundry is easy.

    From the very first day, this treatment center gave us something we desperately needed — hope. My daughter came in unable to even stand on her own. In just 3 weeks, because of their dedication, skill, and genuine care, she is walking with a walker. That progress didn’t happen by accident — it happened because of a team that truly goes above and beyond. Every therapist treated her like more than just a patient. They were patient, encouraging, and consistent. They pushed her when she needed it and celebrated every small victory right alongside us. During one of the hardest seasons of our lives, they became part of our support system. The knowledge, compassion, and commitment this team shows every single day changes lives. It changed ours. I will forever be grateful for the role they played in my daughter’s recovery. If you’re looking for a place that truly cares and produces real results, this is it.

    The rehabilitation programs and nursing staff is so careing and on top of the latest care.and guide your healing and wound care. you need to learn to walk again due to surgery, the phisical therepy staff know the knowledge and understanding each patiences needs and guides you lovenly back to health. Every staff member is attentive in every area and goes out of there way to get you well and going home . If its long term or short term stay , the staff is on top with compassion and works with your dr's and patient health needs. The Dietary Dept makes bland food taste top notch . I highhly recommend this phisicallity They take you places as of monthly trips to local resturants . You are kept busy with all sorts of ongoing things to do .A ecicellent health care place to go to.     The only reason I am giving one star is to write my experience so people is aware of how terrible this place is. The LPNs don't have a clue of what they are doing. The staff is not caring, compassionate or professional at all. If you mention to them about calling 911 to send your loved one to the ER because of the conditions you find him/her, the Doctor and nurses would argue with you, and try everything possible to avoid that. The second time I was going to call the ambulance for another issue, the Doctor made me believe that my Dad's symptoms were due to his health condition and age. He got the nerve to tell me that " if it was his Dad, he wouldn't send him to the ER" ....So I trusted him and didn't call. Two weeks after he got worse, and when I finally sent him to the ER, AGAINST THE DOCTOR'S WILL, it was too late! He had a bad case of sepsis, and a stomach bacteria. He passed away the day after. 😭 Please do not take your loved one there, you will regret it! 😔

    I have just moved my dear mother to West Orange Center for Nursing. I must say, having had my mom in other nursing homes, what sets this nursing home apart from the rest is the level of care and customer service. Someone is always available to help, including weekends. They seem to sincerely care about my mom and her well-being. Their activities staff is creative, caring, patient, and inclusive. My admissions representative, Fatima, has gone above and beyond to ensure my concerns are addressed, as does Jamie, the Nurse Manager, and Nadine, one of the lead nurses. Apologies for rambling; however, when it comes to parents, you want all the feedback you can get. So far so good.🙏🏾😌

    Terrible place to put your mother with the promise she would be taken care of by the Administrator. Nothing had been done to meet my mother needs as she has declined. Lots of promises and no delivery. I wouldn’t leave anyone there that I love and their facilitiy needs the state to come in. Absolutely terrible!

    RIVER'S EDGE REHABILITATION & HEALTHCARE CENTER

    Struggling because I loved Rivers Edge. I had my sister with intellectual disabilities there 3x. They taught her how to walk again after a bad fall. It was a no brainer when I chose Rivers Edge for her to receive hospice when her cancer metastasized since she was already a rehab patient there. Everything was great until hospice took over. I was told by the hospice provider she would be given her pain meds every few hours by syringe. A few days later someone decided to try a fentanyl patch. It didn’t work. I had to fight through hell to get her agitation & pain taken seriously. They kept telling me they couldn’t overmedicate her or she’d get addicted. She was dying.!! There was some kind of miscommunication with the hospice nurse which led to her care delayed. Seemed like no one understood the purpose of hospice is to keep the person comfortable so they can die peacefully. The hospice sent someone back out to see what was going on & was as flabbergasted as I was as to why they weren’t taking her pain & agitation seriously. They kept giving her meds crushed in apple sauce to the point she was starting to aspirate. Seemed as if the staff didn’t understand. I don’t understand what the miscommunication was but I was the one who had to sit there with my poor sister screaming & kicking her legs since the cancer was in her bones. Your nurses continually told me she wasn’t in pain,. She most definitely was since she was literally screaming. I had to witness all that & be told things like “calm down” “you’re too focused on your sister” Even after all I dealt with there, I was still going to recommend your facility until I had an encounter with one if your charge nurses, Shaunte. On 4/23 Tues she was the nurse supervisor at the first floor nurses’ station. She usually worked upstairs so maybe she was filling in. We had questions about medications my sister was getting & when I said hospice was coming back that night to see what they can do, the nurse rolled her eyes & said “they been here already today” I was shocked at the eye roll but needed to be civil. I said “well they have to come back out & make recommendations” she said “well they can make all the recommendations they want but it doesn’t mean we are going to follow them” No reason for her to talk like that to me. I had nightmares that night about my sister screaming in pain & none of your people caring. I was sick. Hospice came & made the recommendation (still as confused as I was as to why it had to get to that point) which were implemented the next day. My sister only had one day of continuous comfort. She died the next day on 4/25 I still feel traumatized by her pain & agitation not being taken seriously & the fact I trusted your facility to make her comfortable. Then to have that charge nurse talk to me like that not even 48hrs before my sister died. Shame on you for having such a person as a supervisor. The rest of your staff are well meaning & kind. Out of the 100 or so nurses I’ve dealt with during my sister’s illness not one was rude & unprofessional to me like that.. Does your facility not understand the gravity of when a patient is in hospice & what the family is dealing with?. The CNAs, receptionists, most of the nurses, Alisha & kitchen staff were all great & empathetic & wonderful which led to me being shocked at having to deal with someone so nasty like that. My sister was dying. Another recommendation is to make your activities director know who is in hospice & who is not. She came into my sister’s room & chided her for not getting out of bed & doing things. It’s her job to get patients up, I get that. There shouid just be some kind of note or symbol. At that point, my sister was not out of bed for 5wks & was pretty much nonverbal since the cancer was wreaking havoc in her brain. If she tried to get up she would’ve collapsed due to muscle atrophy. You guys need to make a notation or something. If she got up due to being confused she could have fallen & with cancer in her bones that would’ve been awful.

    My mother who is 74 years old had major back surgery later January 2024. She was a patient at East Neck for about 30 days. This was her 2nd stay in a short term rehab, but the most significant. Taylor in Admissions made sure she was transferred from the hospital to EN with no issues. Dr. Balot was in frequently managing her medical issues and concerns as well as pain. Megan in social work communicated her plan of care to with me and coordinated a safe discharge. Also, the rehab department was amazing! She was up and walking in no time. My mother had no complaints and is extremely thankful for the staff at East Neck!!!

    I have been in many rehabilitation facilities in past and recent years, and one person stands out among all. Yvette Kemp Makes a patient feel very special. She goes the extra mile to make sure one is comfortable including cleanliness, and a large amount of positive mental support. I’m partially paralyzed and it did not take her long to be aware of my special needs, and made sure that I received the extra attention. From my observation she takes her job seriously and always with her friendly personality. Thank you Yvette for all your your special care.

    I just recently admitted my mother to this facility. When we toured, I saw everyone (staff and patients included) smiling and moving about the entire building. The morale felt great. Fatima in admissions was kind, helpful, and she made everything so easy for us. Sue (possibly admissions as well) was quick to respond, eager to help in any way, and extremely sweet. Additionally, the two people I’ve encountered at the front desk have been nice and helpful as well! Finally, the nurses have been present frequently so far and they have been very nice.

    I cannot say enough wonderful things about my experience at this facility. From the moment we called East Neck to see the place and entered the building, we were met with warmth, compassion, and a level of kindness that immediately put our family at ease. The staff truly go above and beyond and you can tell they care. the nurse, CNA, therapist, and team member we have encountered treated my loved one with patience, dignity, and respect. They took the time to explain each step of the care process, answer questions, and offer encouragement on difficult days. The rehabilitation team deserves special mention — their dedication, expertise, and positive attitude made all the difference in the recovery journey. Watching the progress from admission to (almost at) discharge has been remarkable, and we are so grateful for the support. In a world where healthcare can sometimes feel rushed or impersonal, this facility stands out. They treat patients like family, and that level of compassion truly matters. We are forever thankful to the entire staff for the care, kindness, and comfort they provided. We are so glad we came to tour this facility and met the people we did from the start. We thank god every day for those who gave their numbers and answer our questions any time of the day. Highly recommended for anyone looking for a skilled and caring place for their loved one.     The nurses are great - But when An Er Doctor sends you home with a Very high blood pressure - it’s crazy and when the CNY cardiology department does nothing to investigate the cause of Months of High Blood pressure averaging 180/110 - Then you know it’s not safe to go to those places . Since June I’ve had cardiac Symptoms- and since July have had several med Changes - And the very high blood pressure has made Last year - I had an allergic reaction to something- as they were rolling me out of the Er heading for the observation room up on a floor. They asked me in transit if I wanted to be resuscitated?!! I’m said why and the nurse s said well because there is only one doc in the ER . I said Yes Please id like a Jump start The Docs Act like they know everything but Don’t even Know How To “ PRACTICE “. Its embarrassing to have to need an eR and know LCGH is the closest one to get to . They have no compassion don’t have common sense to quickly evaluate. They don’t listen to everything you tell them and then after seeing a CNY Doc a few times - Not one Doctor has done what they should be doing . I’m speaking up bc No One has done Extensive testing to find the cause. And given I’ve had a carotid Brain Aneurysm with a stent and coils .... you’d think after weeks of throwing pills at me - changing them , Someone would be serious and Do their Job before I can’t represent myself anymore . No one should be this sick for this long without a doctor following thru to determine the cause . Good thing I’ve kept all my notes .

    Recently the nursing home relocated my mom from 3rd floor to the 2nd floor and about 2 wks ago she was relocated again to the 5th floor. So far we are very pleased with the CNAs and its residents. They both look out for my mom's wellbeing. All are very attentive and care for one another. My mom is kept clean and so is her room. Therefore, the family is very pleased with our Mother's upkeep. I would like to express my thanks and today, 5/7/25, to all of the staff of Downtown Brooklyn Nursing and Rehabilitation, but double thanks to CNA, Ellis. Thank you for ALL you do and the phenomenal job you do daily. Your caring and kindness does not go unnoticed. My family and i appreciation you. Today, 4/25/25, i would like to express my thanks and appreciation to Downtown Brooklyn Nursing and Rehabilitation and its Registered Nurses, Certified Nurse's Assistants and Occupational Therapists for taking good care of my Mom even as her mental condition progressives. Today, 4/10/25, I would like to express my thanks and appreciation to CN, Augustine. Thank you for your special care to my Mom, R. Acosta. I must say, the entire staff are always cheerful and willing to assist at any giving time. They are very attentive to each patient's needs. Again, the facility is spotless and very well kept. Keep up the great job to the facility leaders and its staff. It's been approximately 3 wks my mom was admitted to this nursing/rehabilitation, and the family is very pleased and happy with the service my mom has received. From the CNAs to the Occupational Therapists, Nidhi and Kermit. Both are exceptional in their task and caring to their patients. The facility is spotless including the beds, and the patients are cleaned and seemed happy. I would like to express my thanks to nutritionist, Ashley for taking the initiative and listening to the patient's concerns and adjusting their food intake to their preference. I would like to express my thanks and appreciation to CNA, Grayson. Thank you for all of your assistance and all that you do to make the day run smoothly. Keep up the amazing job that you do. I would like to express my thanks and appreciation to CNA, Tanesha Sutherland, she's a true gem and always pleasant, attentive and very helpful. My rating for DBNRC is A+. My mom's room is spotless and so is the dining room and the entire facility. Thank you to all the staff for your assistance, politeness and services. Although it's too early for a detailed review. My family and i are very pleased with how clean the premise is and how attentive the staff and nurses are.

Nurses are rude, fatalities is dirty just so much.My parent was filled here and took forever to send them to ER.The next day I went to get their things it was packed and was about to be thrown in the trash.Do yourself a favor and look elsewhere

    Staff involved in basic patient care was inept. During end of life care, assistive staff lacked respect, was yelling to coworkers outside of the room and laughing. Therapy staff remained affectionate and respectful but they weren't the ones providing regular care. A lack of empathy was shown by assistive personnel.

    CHARLOTTE BAY REHAB AND CARE CENTER

    This place is awful and I believe fosters aa poor attitude related to patient treatment. My wife has been at this facility 3 times (Unfortunately I could not control her going to some other facility) She has been a patient at this lousy facility three times, Once for 6 days, once for 7 days, once for 36 days. Each time due to the negligence of this facility she had to be transferred to the ER for the same issues as they could not properly give her the correct treatment to ensure she did not end up in the ER. The most recent time she was transported she was admitted with acute respiratory failure, Influenza B, Pneumonia, High lactic acid, a UTI and several large pressure wounds for which I was never informed of and she developed during her last stay. Also, my wife had 1 personal item, her prescription glasses. When transported to the ER this facility could not even make sure her glasses went with her, Unbelievable. Recently, she experienced respiratory difficulties for about a half hour. I rang the nurse call. After 5 minutes when no nurse came, I went to the nurses desk to explain the concerns and asked for a nurse to please come down to her room. A NURSE DID NOT SHOW UP UNTIL 30 minutes later. How unprofessional. During my visits, I have experienced windows of up to 4 hours or more before a nurse stopped by. My wife is unable to speak and press a call button by herself, so getting frequent visits is very important. I find that the treatment at this facility is inadequate and personally a danger to patients. I truly don't believe this facility really cares about patient well-being. Fair to say we will never be going back to this facility and I am in process of submitting formal complaints with the proper regulatory organizations to ensure they are investigated.

    Do not send your family here. My grandmother has been there for a few months and she has been in and out of the hospital because they aren't taking the proper precautions. They know she is a fall risk and yet, they don't seem to care. They have the right technology to prevent accidents from happening, but most of the nurses there are so ignorant that they won't use it.

    When i came to the facility, i was very happy with the rehab staff. My therapist, Simon, worked very hard with me and always gave me fun and challenging tasks. Even though i was away from home, I was comfortable staying here. The staff was friendly, the rooms were clean, and my bed was always made. The one problem, the toilet in my room was broken and took a couple days to fix, and i always had a bathroom to use. Overall my experience here at downtown brooklyn nursing home was good, clean, all the people and workers are clean. I would recommend friends, family or anyone come to DBNR for short term or long term care.

    If I could give zero stars, I would. Absolutely horrible. My loved one was there recently and he contracted rashes and sores due to neglect. We had to practically force the employees to do the bare minimum (shower, change his clothes, walk him around). The place was filthier than the hospital and staff were hard to find. Everyone was clearly under cared for. It was difficult to get him out of the facility due to the employees’ lack of communication and effort. It took them days to get my loved one his medications which caused him more problems! I wouldn’t wish this place upon my worst enemy. If you care about the well being of your loved ones DO NOT take them here!

    My mother was in this facility. She woke up one morning for breakfast. She had a headache so she told the nurse that she was going to lay back down which was around 9am. When lunch came no one checked on her or even woke her up to take her beds. Long story short, not one nurse or aide try waking her up all day or night so she did without medications or food. The next day around 9am a nurse went into her room to wake her and my mother was unresponsive. My mother had a stroke the prior morning. She laid there for over 13 hours and no one checked on her. When we got to the hospital she was on life support, they told us there was nothing they can do for her. My mother was brain dead because they waited too long to call an ambulance. We had to take my Mom off of life support and she passed away. And not to mention, they weren't giving her the blood thinner that she was supposed to be taking.

    My mother was only there from 11 pm last night until 8 this morning. She called me at 6 am and told me to get her out of there. No one would help her or assist her to the bathroom. She was told to go in her diaper. My mother is not accustomed to wearing diapers and not using a commode or toilet at home or when she was in the hospital. The place is not very clean either. I live rate this place 0 stars.

    I visited several times to see my loved one. The entrance was inviting. Check in process was easy. The receptionist was friendly. Lots of daily activities. My loved one raves about her doctor, PT and OT. They are very supportive and encouraging. Her room was large and clean. It's really cold in the room. Linens are changed nearly every day and bed is made. Concerns about the dietary restrictions were addressed but not rectified. Food missing off her tray. Concerns about the toilet leaking urine on the floor have gone unaddressed for over a week according to my loved one. Some of the GNAs are sarcastic and doing the minimum. My loved one asked staff if they could have a sheet on the floor to soak up urine but was denied. Her footies get wet and when asked for a clean pair, was told she was issued one pair on admission this she would not receive a clean pair. This is concerning 😟. Otherwise, it's been a positive experience for my loved one who is looking to come home.

    When I placed my mother in Downtown Nursing and Rehabilitation Center (DNRC) I did not know what to expect. To get to the point I am really quite pleased with them. The first one I met there was a lady from admin ( Gabriella), she was so welcoming, genuine, sympathetic to the needs of my family. As I was waiting for the finance officer Tanisa (who was extremely helpful and kind), I noticed how the staff there were treating and talking to the people under their care, (as if they where there own grandparents). The facility was clean inside and out. The facility was secure. There was another lady from administration (Sabrina), She was really understanding and calming. I have to mention the staff at the reception desk, they are always pleasant and efficient no matter what time of day I see them.

    Initially, we were hesitant to consider any type of rehabilitation facility due to past experiences. However, after learning that OH ARI provided three hours per day of physical and occupational therapy, we decided to give it a try—and we’re so glad we did. The staff, including but not limited to Karyn (PT), Raquel (OT), Ash (Nursing), Jeff (Nursing), and Anna (Case Management), were truly amazing. Each one of them went above and beyond to ensure our needs were met with professionalism, compassion, and dedication. One of my primary goals was to return home as quickly and safely as possible. Thanks to the incredible support and encouragement from the team, I was able to exceed expectations and discharge earlier than anticipated. The facility itself is outstanding. Should the need for rehabilitation arise again, I will absolutely choose OH ARI without hesitation.

    Leaving 5 stars for the new wide bore MRI machine. It’s a bit loud, but they all are. I have claustrophobia but handled this easily. Julie broke it up in short timed segments and explained what was coming. She’s an excellent MRI tech. Don’t put it off if you need one done!

    My mother was in rehab here for 5 months & had excellent care. The CNA’s Amber, Josh & Angela treated her like family and were always there shortly after she called them. She never had to wait on anyone. Nurses Penny & Rhonda took good care of her also as well as all the others whose names I can’t remember. The staff made her stay a pleasant experience. Monty & CeCe are awesome & so were the other therapists. Food was not good
