"My mother was transported to Fairlawn recently after surgery for a short stay rehabilitation. I’ve been so impressed by the level of care and how clean the facility is kept. The downtown Gresham facility she was in the prior year smelled and the care staff sat at a desk most of the time playing on their phones. That’s not the case at Fairlawn where I have not seen anyone just sitting. Also a shoutout to Todd in therapy who truly cares and explains why things are happening in an easy to understand way. I’ll be updating the review as my mom recovers. Update 12/23: resident food is not very good. Most of the staff is awesome, shoutout to Pema who truly cares about my mother. There was an incident with PT who left my mother in pain sitting in a wheelchair for over one hour. The facility did address what happened and PT has improved. I would suggest when requesting special needs or equipment to document the date, time and persons name. I found we have to make requests up to four times before results. Also, if a procedure is ordered, stay on top of it to insure it’s done timely and ask for the results to make sure a staff member is aware if there is a problem. The place is very clean and in the holiday spirit."
"My Mom was discharged to this rehab facility from Mt. Hood Legacy. She had a hospital stay for weakness and nausea after her last round of chemotherapy; her bloodwork and all tests' results were positive upon discharge and her lung tumor gone! Her discharge paperwork and instructions were very clear. She was supposed to begin OT & PT immediately and return to her home asap. We had been through a lot in the last four months but everything that happened after she got to this place was an absolute Nightmare. Do not let anyone you love be put into long term care in this place! Read on if you need more devastating details. There were no beds available in the short-term rehab wing so they put her in the long-term care wing. Rehab hadn't started after a week... she needed to have her care plan meeting first. Still not sure why this was required has they had all the details and OT & PT instructions from her doctor. She then caught covid in there (most likely from the staff as none of the family got it, even visiting her while she was sick) the 14 day quarantine rule took over before ANY services could begin. There is very poor communication. There is not enough staff, several staff members do not do their job. Administration does not respond. Therapists canceled services stating insurance would not cover anymore, I filed a claim and insurance still covered everything she needed. Medication aides give meds early; the in-house MD over medicates. More time and red tape as my Mom declined on a daily basis. Pain from being left in bed. Bed sores. Staph infection. If I knew... what I know now! I would have taken my Mom home!!! She didn't want to be a burden and this breaks my heart. It is so sad and completely wrong that this is a learning experience for families... even having a "care manager" and a case worker there is so much to learn and navigate, Especially when no one does their job. Advocating for your loved one becomes a job in itself. Many more unprofessional negative things happened But I will end this by saying please do not support this place! They should be shut down."
"Never ever send your loved one here, if you truly care about them.. my mom was there from July/ August until she passed at the end of August. They were HORRIBLE to her. She was terminally ill with cancer, they wouldn’t go to her call light when she pressed it. Sometimes it would be upward of 2 hours until they would check on her. Many times she would call me because they would be ignoring her, I would try to call the front over and over with no answer, but when I would get there all the “ staff “ would be sitting next to the phone hanging out. She would call me sobbing in pain, stuck in one position that was hurting her ( as her mobility got less and less as she declined ) or hurting because she was in a solid incontinence brief for so long she get bleeding rashes. The way they would talk to her was outrageously disgusting, and they would act like she such a burden whenever she would need something ( which was really rare ). They have no compassion whatsoever. So please never ever send any loved one to this place, because this was rough to watch my mom go through at the end of her life.."
"My mother has been exceptionally cared for over the past 10 months. The staff here really cares about their residents. So many wonderful CNA'S and caregivers. The front office staff is equally amazing. Thomas in admin and Marianne, the activities director, are the advocates you need for your loved one. We came here, completely overwhelmed by the Medicaid system and they have helped me navigate this new world while giving my mom the best of care. She's made friends, participates in the many activities and has gained weight! She wasn't caring for herself and eating regularly, Fairlawn has properly fattened her right up, just don't tell her that 😁. I love you all and will forever be grateful for the love and care you have shared with my family."
"I spent 3 weeks after a car accident. It was a very nice place to rehab.Everyone there we're super sweet and very helpful.Thank you Fairlawn."
"Fairlawn Village is a disgraceful, disgusting establishment that ROBS its patients of their money, their dignity and their right to good medical care!!! My mother is a liver transplant patient who is also diabetic and have multiple sclerosis. They constantly left my mother in her soiled briefs for MANY hours, refused to give her her LIFE DEPENDENT medications on a proper schedule, malnutritioned her by not feeding her foods she could actually eat and often the food was under cooked and they wouldn’t fix it. They even stopped allowing g her to buy her own food because no one wanted to help her prepare it. Upon move out, they refused to send her with all of her medications that, again, her LIFE DEPENDS ON, that HER insurance paid for. Anti-liver rejection medications, insulin, needles, multiple sclerosis medications, her anti-depressants, all her supplies… she has had nothing but infections, multiple emergency room visits including the ICU for diabetic ketoacidosis that they REFUSED to acknowledge, and now she has SEVERE open wound sores on her that they never tended to. She has massive hematomas on her arms from her liver transplant not getting the proper medications. They forced her to stop seeing the primary care doctor who has been with her since her transplant in 2004, to see their “in house” doctor who never did a dang thing for her and was constantly unreachable. They always called me trying to make her sound crazy, when in reality they wanted to cover their own tracks on their bad care. She may have MS but she is VERY coherent and aware, she’s not a vegetable that can’t advocate for herself. They also stole her card information and tried to charge an ADDITIONAL $3,000 that she didn’t owe, which prompted social security to shut her card off AGAIN for a third time! I am going to make sure this company is completely destroyed by legal fees and state/federal violations. Mark my words. Everyone in this establishment deserves better. People go there to die, not to get better. “Rehabilitation” is an absolute joke. If you have a family member here, get them out ASAP and come equipped with attorneys and a sheriff to ensure they give you EVERYTHING that belongs to your loved one so they can’t rob them of what is theirs. Shame on you, Fairlawn!"
"Please, why is there a lock on the restroom inside the entrance???!!!! I couldn't find anyone to help me. I wanted to visit my grandmother and for her to see her new grandson but because of the restroom lock issue we had to leave. This was quite a drive from home. To the owner, please be mindful of the family members and friends visiting their loved ones. It should not be so difficult to use a restroom"
"We have a family member that’s been at Cascadia Fairlawn for several months we are extremely happy with the care she has been given. They have been very attentive to her and needs and the staff is always friendly and very kind to her. We would highly recommend this facility for short or long term care!"
"My Mother had voluntary hip replacement surgery in March 2022. She went into Fairlawn rehab from the hospital after successful surgery and was misdiagnosed with covid, isolated, left in soaked depends with call button on floor. They failed to put rented machine on her legs to prevent clots. We put it on her. They said she had "failure to thrive " for failure to eat and depression. She came down with a blood clot and GI bleed and was sent to a hospital that had no GI doctors on staff. After speaking to Fairlawn staff the ER doctor suggested palatiative care. She lasted 6 days and died 1 mo. after elective surgery. Wouldn't send worst enemy to Fairlawn rehab."
