Cottage Grove Place maintains higher-than-average total nursing hours but faces challenges with high registered nurse turnover, reliance on contract workers, and a recent history of safety-related inspection findings.
The data indicates a facility in transition, with a decline in the overall federal quality rating from five stars to two stars over a two-year period. While the facility maintains a high total staffing volume of 4.48 hours of nursing care per resident per day—exceeding the Iowa state average of 3.83 hours—registered nurse turnover is notable at 50%. This means half of the registered nursing staff left within a twelve-month period, which is higher than the state average of 41.7%. Additionally, data shows a reliance on contract staffing for registered nurses at 36.4%, suggesting that more than one-third of the registered nursing care is provided by temporary agency workers rather than permanent employees.
Inspection records reflect significant oversight concerns, particularly regarding resident safety and staffing levels. A 2026 finding documented a failure to prevent accident hazards with a 'harm' severity rating. Furthermore, a 2024 citation noted that the facility did not provide enough nursing staff daily to meet resident needs. While the facility currently holds a four-star rating for staffing, its health inspection score has moved from four stars to two stars. The facility has not incurred any federal fines or payment denials in the recent period, but the high number of complaint-linked findings (16 across recent cycles) suggests a pattern of reported issues compared to facilities with fewer inspection citations.
A recent inspection on January 6, 2026, identified a failure to keep the facility free from accident hazards, resulting in a severity level G rating (actual harm that is not immediate jeopardy).
Records from September 2024 indicate the facility failed to provide enough nursing staff every day to meet resident needs and did not consistently have a licensed nurse in charge on each shift.
While the facility uses an average of 4.48 hours of total nursing care per resident per day, which is higher than the state average of 3.83, the weekend staffing levels drop by approximately 10% to 4.04 hours.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"What a Delightful Day at Cottage Grove Place! Today I had the honor of being invited to Cottage Grove Place to participate in a panel—and I’m so glad I was invited! I learned so much about the community that I didn’t know before. There are so many wonderful features and benefits that I think people often overlook. One thing that truly stood out to me was how happy everyone seemed—both the residents and the staff. There’s a genuine sense of warmth and community, and the people who work there are absolutely delightful. As a bonus, I was treated to lunch, and let me just say: the creamy pesto chicken pasta was absolutely delicious! If you’re considering a move to a senior living community, I definitely recommend putting Cottage Grove Place on your list. It has so much to offer, and I came away truly impressed."
"Wonderful! Not only a beautiful place but also friendly and hard working staff! Fun activities and so much to offer! The residents are all very friendly and go out of their way to make people new to Cottage Grove feel welcome."
"The 2 seminars I attended were both excellent. Gina on down-sizing was super, informative and entertaining. The lady from AARP was both knowledgeable and very helpful."
"There is a magic in this place. I had a lucky moment in life, when I was fortunate enough to be able to experience this magic. It changed me. Made me want to do better, be better. This magic is now a part of me. Guides me in everything I do. I am grateful beyond measure."
"I have had experience with Cottage Grove Place through the nursing care department. We have also been invited to participate in activities after I got home. The staff is always very friendly and helpful. The residents speak highly of Cottage Grove Place through Place as well. Thank you for including us in your invitations."
"I have had the pleasure of visiting Cottage Grove Place hundreds of times over the past 8 years, and I must say, it exceeded all of my expectations. From the moment I walked through the doors, it was evident that this community is not just a place to live—it's a place to thrive. The staff at Cottage Grove Place are truly exceptional. They go above and beyond in every way to ensure that the residents feel comfortable, cared for, and valued. Whether it’s the friendly greetings, the attentive care, or the genuine smiles, you can tell that the staff here really care about each and every resident. It's not just a job for them; it’s a calling. The facilities are top-notch, offering a wide range of activities and amenities that cater to all interests and needs. Whether it's joining a fitness class, attending a social event, or simply enjoying the beautifully landscaped grounds, there's something for everyone here. The community has a warm and welcoming atmosphere that makes it easy to make friends and feel at home. One of the things that really stood out to me was the emphasis on personalized care. The staff take the time to get to know each resident, their preferences, and their needs. This individualized attention ensures that everyone receives the care and support they need, while still maintaining their independence and dignity. I also appreciate how the community fosters a sense of belonging and inclusion. There’s a strong focus on creating a family-like environment where residents feel connected and supported, both by the staff and by each other. It’s clear that Cottage Grove Place is committed to providing not just a place to live, but a place to enjoy life to the fullest. In short, Cottage Grove Place is a wonderful community where residents are truly cared for. If you’re looking for a place where your loved ones will be treated with the utmost respect and compassion, I highly recommend considering this community. It’s a place where care meets compassion, and where residents are treated like family."
"My mother in law lived at Cottage Grove Place in skilled care, Assisted Living Plus, and the health center. Staff everywhere were professional, patient, and kind as her dementia progressed, even when she was difficult to deal with. Many staff members have worked there for several years, which speaks to the friendly, caring environment at CGP. I would highly recommend Cottage Grove to anyone seeking professional, compassionate care for themselves or a family member."
