Records indicate this facility is a Special Focus Facility with an abuse icon, high financial penalties, and health inspection findings that significantly exceed state averages.
The data indicates that this facility manages a profile of high regulatory oversight and significant enforcement actions. Records show that the facility has been designated as a Special Focus Facility, a classification assigned by the Centers for Medicare and Medicaid Services to nursing homes that demonstrate a chronic pattern of care failures. In the most recent full inspection cycle, investigators recorded 50 health findings, including a 'Severity J' citation—the highest level of immediate jeopardy—related to failure to provide adequate supervision to prevent accidents. This deficiency count is more than four times the Missouri state average.
Furthermore, the facility’s staffing data is historically incomplete, though recent snapshots indicate total nursing care of 2.17 hours per resident per day. This remains below the state average of 3.47 hours. Enforcement data reveals five major penalties in the last three years, including three fines totaling $275,024 and two separate periods where federal payments were denied. The presence of an abuse icon on public records further indicates that federal regulators have identified issues related to resident protection and safety within the facility's recent history.
The facility is designated as a Special Focus Facility, a status reserved for nursing homes with a history of persistent safety and care issues.
Health inspections recorded 50 deficiencies in the most recent cycle, significantly exceeding the Missouri state average of 11.9 findings per cycle.
Public records include an abuse icon, signifying that the facility has been cited for incidents involving abuse or neglect within the past year.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

"This nursing home truly feels like a second family. From the moment you walk in, you’re greeted with kindness, compassion, and genuine care. The staff goes above and beyond to make residents feel safe, respected, and valued every single day. The nurses and caregivers are attentive, patient, and clearly passionate about what they do. Communication with families is excellent, and it’s comforting to know loved ones are in capable, caring hands. The activities keep residents engaged, happy, and social, and the environment is always clean and welcoming. It brings peace of mind knowing our loved one is not just cared for, but truly cared about. I highly recommend this facility to anyone looking for a place where compassion, professionalism, and dignity come first."
"Our family is so grateful for the care my grandmother received at this long-term care facility. The staff was consistently kind, friendly, and compassionate. They treated my grandma like she was part of their own family, and that meant everything to us. Knowing she was cared for with dignity, patience, and genuine love brought our family great comfort. I truly appreciate everything they did for her."

"The new administration and Don are doing a great job improving the building and the environment And new staff is doing great job with the resident care The improvements here are just fantastic. You would be amazed at how it is now. The residents are getting getting taken care of by the CNA‘s and nurses we have good ones and they are loved and taken very good care of"
"For the time being, this place will be getting three stars due to the friendliness of some of the employees. What I don't like about this place is, my mom has been here for a day now. She informed me that she had to wait two damn hours before she got changed. No resident(my mom will only be there until her therapy sessions are complete) should have to wait so long in soiled bed linens. Id like to see some agressive attentiveness when it comes to my mother being there."
"My grandmother went to Willow Care the last 2 1/2 weeks of her life. She was almost 101 and recovering from Shingles. The staff of Willow Care was extremely compassionate as we watched her slip away. It was very apparent that the entire staff was not just doing a job, but were there for the right reasons. They went out of their way to bring us anything we needed. They showed us kindness and provided support, more than I ever expected. They were also very kind to grandma, squeezing her hand or giving loving pats. It was a difficult time for our family, and they helped us cope."
"The staff is well equipped, caring and understanding. This place has a clean environment with extra entertainment such as live bands for the patients to listen to.They seem to be short on help on the floor though."
"Appreciate the new leadership and the current staff at Beloved Keep up the good work."
