Based on public records, this facility is characterized by a high volume of health inspection findings, a federal abuse icon designation, and extremely high nursing staff turnover despite strong quality scores for long-term residents.
The data indicates significant challenges regarding regulatory compliance and personnel stability at this facility. Records from the Centers for Medicare and Medicaid Services (CMS) reflect an overall rating of 1 out of 5 stars, with the health inspection history revealing a significant volume of findings. While the most recent inspection showed 8 findings, the prior cycle recorded 62 findings, including 33 related to complaints. Financial penalties have been substantial, totaling $176,644 across six fines and two payment denials in the last two years. Furthermore, the presence of an abuse icon on the facility's public profile highlights documented safety concerns.
Staffing levels present a complex profile. While the facility provides 4.16 total hours of nursing care per resident per day, which is near the state average, the allocation of that care is heavily weighted toward licensed practical nurses and certified nursing assistants. Daily registered nurse care is 0.28 hours per resident, well below the California average of 0.65 hours. Additionally, nursing turnover is very high at 67.7%, meaning approximately two-thirds of the nursing staff departed within a twelve-month period. Although the facility does not utilize contract or agency staffing, the high turnover and lower-than-average registered nurse hours indicate a lack of long-term consistency in the professional care team. Quality measures for long-stay residents are rated at 5 out of 5 stars, suggesting better performance in chronic care metrics, while short-stay measures remain lower at 2 out of 5 stars.
The facility has been flagged by CMS with an abuse icon, indicating a history of incidents related to resident safety and mistreatment.
Registered nurse care hours are 0.28 per resident per day, which is 56% lower than the California state average of 0.65 hours.
Total nursing staff turnover is 67.7%, significantly exceeding the state average of 38.1%, with registered nurse turnover reaching 75%.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"My grandma is here and has been her for the past month. The staff is very caring and always making sure my grandma is doing well. They are very attentive. I couldn’t ask for a better place to have my grandma rehabilitate. This place will always be my first choice if need be."
"They gave good quality care of my father even though we had alot of family dynamics. Therapy is amazing and they were very attentive to his needs.."
"Thank you so much to all the staff at Maple Healthcare Center. I visited my grandmother this weekend from out of state and it was fabulous to see how happy she is here. It is clear she is well taken care of and this facility is much better then her previous nursing home she was at."
"my uncle stayed here because of a stroke and they took such good care of him. my uncle has been staying there for a while and it’s just amazing to see how good they take care of their patients! it’s gotten much better since they’ve had new management"
"Maple care center gave my mom the best experience and care. The staff was so sweet and so on top of her medication, food, high gene and needs. The residents are all very nice too. They are true hero's always welcome me and my family to see my mom or talk to her on the phone at anytime and answered all my quetion and concerns too. All residents are none covid patients our visitations wore behind a window witch was ok because we want to keep my mom and all residents and staff safe. Thank you!"
"I give this place 2 stars only for some of the C.N.A's and activity people who genuinely cared for my grandfather. I could give 0 stars to office staff for not not really assisting me with paperwork upon his release. Upon my grandfather's release they did not describe to me the level of care he would need. I figured he was more independent. I am upset with doctors who over medicated him to the point he has drug induced Parkinson's. I had to wean him off several medications. From the time he was admitted to his release, his mobility declined rapidly. My grandfather suffered a manic episode during his stay there only to have his medication increased to the point he only wanted to sleep all day. I did request that they give my grandfather information as to which medications he was on and that they let him know about his finances. Every time i called staff it seemed like a hassle. He wanted to give me some money at one point which took several months for them to send me a legit check. I emailed my concerns only to be pushed to the side.. They misplaced and lost his things including his dentures and glasses. They were not attentive to other patients walking in his room taking his belongings. My grandfather is a veteran and i received more help from the VA hospital than i did at this nursing facility. It saddens me that they did a lot of damage to my grandfather until i was able to care for him. I feel this facility should be investigated especially during covid. They have most patients in bed all day. Please keep track of your love ones medications, finances, and belongings if you send them to this facility. There are only a few here who really care. Mostly cnas' who been there for years. When I did visit i noticed staff more on their phone than attending to their patients. I really wish i could of done more for my grandfather while he was there because being there affected his health greatly. If your loved may have a disease which causes them to act out please be aware of medications they give them. Some of These medications are very strong and cause a lot more side effects. Most nursing homes don't want people too active because they don't want to be responsible for falls. My grandfather experienced falls as well. One in which caused injury to his back. He says he woke early to shower and had an fall. I'm wondering where staff was at that time also. I am just so upset with this facility. Please be on top of the care your family receives I feel for the ones who don't have anyone, i can only imagine the type of care they receive. I did notice favoritism at this facility as well. Staff who pick and choose to give quality care to. Some of the people there were rude as well. I think they wanted him released as soon as they could before there was a complaint. He really got tired of being there..before his medication increase he was very vocal about what bothered him. They changed him soo much its sad. They can't give me back my grandpa the way he was before the damage. Shame on this facility."
"They have a thief working their. My mom just got discharged this morning, once she got home she was looking for her money and it was missing She had $500, half her rent she forgot she had it in her wallet. It was their when she was admitted from the hospital. She hated the food and some of the staff especially the night shift nurses, they place the nurse call devise out of reach as to not be bothere. She liked the day shift staff most were friendly and caring"
"Please whatever you DO NOT BRING YOURE FAMILY MEMEBER HERE they cover for each other when doing shady activity my father was there a year ago and I was told it was a locked facility they cannot get out however I wanted to take him out he wanted some fresh air I’d bring him back they said they needed the dr to approve the dr never got back to them so we were unable to then my father got a spider bite it got infected apparently they didn’t “know “ then they changed it to we saw it but didn’t see it was bad he never said anything ?! There supposeuvly always check patients then until I complained and my father spoke up because it became bad they decided to take him the the hospital after days passed he returned and had a fit he wanted to get out and said they released him because he was having a fit but couldn’t allow me to take him out on visits supervised by me and it was considered a lock facility were he was not going to be allowed to leave in other words if u want ur family member to get out have them have a fit a huge fit where the staff can’t take it and garuantee they will be let out ."
"Horrible place to keep your loved ones. Staff simply don't care! They are rude and inefficient. I had to forward a concern from my brother ( inpatient)to the staff because they just don't care to listen to the patients."
"Facility is clean, staff is friendly and caring. Facility is odor free."
