This facility holds a one-star overall rating characterized by high staff turnover, significant reliance on contract labor, and recent high-severity inspection findings related to resident safety and abuse.
Data from the Centers for Medicare and Medicaid Services (CMS) indicates a significant downward trend in quality indicators for this facility. The overall rating has declined from five stars to one star over a 21-month period. This decline is reflected in the staffing metrics, where nursing care hours per resident per day have dropped by approximately 12.5%, currently sitting at 2.9 hours compared to the South Carolina state average of 3.86 hours. Registered nurse care is particularly low at 0.30 hours per resident day, which is less than half the state average of 0.63 hours.
Further data points to instability in care continuity. Total nursing turnover is documented at 71.2%, with registered nurse turnover reaching 83.3%. Additionally, nearly 29% of the nursing workforce consists of contract or agency staff, which is well above typical health standards and suggests a reliance on temporary personnel. Inspection records from February 2024 identified 10 deficiencies related to complaints, including a high-severity finding involving the protection of residents from abuse and neglect. While long-stay quality measures remain high, short-stay measures were rated at the lowest possible level by federal regulators.
CMS records for early 2024 documented an 'immediate jeopardy' finding involving failure to protect residents from abuse and neglect.
The facility reported nursing staff turnover of 71.2%, which is significantly higher than the South Carolina state average of 47%.
Recent records show $85,163 in federal fines and a 24-day denial of payment from Medicare/Medicaid.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"Ms. Gloria Brown is truly a gem at Forest Acres Acute. As the Activity Director, she goes far beyond what is expected to make sure every resident feels seen, valued, and included. You can tell that she doesn’t just do her job — she genuinely cares. Ms. Brown brings so much warmth, patience, and positivity into the building. She takes the time to get to know each resident and creates activities that make them feel engaged, happy, and alive again. Whether it’s through games, music, conversations, or simple one-on-one moments, she makes sure no one feels forgotten. What stands out the most about Ms. Brown is her heart. She treats residents with dignity and respect, and it shows in how they respond to her. You can see the joy on their faces when she walks in the room — that says everything. Forest Acres Acute is truly blessed to have Ms. Gloria Brown. She is not only an amazing Activity Director, but an incredible human being who makes a real difference every single day."
"The cNA’s were fantastic and some of the nurses were good. Most of them had their name tags turned around so you couldn’t see their name. If you ask them for anything besides medication, they would make you wait ACNA to come. There were multiple times that I ended up being incontinent because they didn’t get the message . The part I have the most complaints about is the social workers. I wanted to get an extension and they kept telling me they would bring me the papers and leave my room and never bring the papers. This happened three days in a row and I missed my opportunity to appeal. When I tried to file Extension, they did the same thing with saying they would bring the paperwork and they didn’t. This also Happen three days in a row. The third day, they made me go to the ER by ambulance because in my notes they saw that I had terrible symptoms the week before. They address them and that was it. Or so I thought. They ended up keeping me in the ER it was too late for me to appeal again On the day of my discharge, the Appeal hadn’t been completed. I asked them to let me Completed. They denied that request and told me that I need you to be ready to Minutes or they were gonna set me out in the lobby and call the police because I was trespassing. They finally let me stay, But at 1:30, they came in my room and told me I had to be out by two. I told them that no one would be home, but that didn’t matter to them . The CNA came in and started stuffing all my stuff and some of their stuff in trash bags. They put me on the ambulance and I had told them that someone wouldn’t be at my home till between two and three. I ended up sitting in the ambulance for 45 minutes. Before someone got home. And then the cherry on top… My appeal was approved and I could’ve stayed. After I came home, I got a visit from DSS. Forest Acres rehab had called in stating that I didn’t want to go home because work was being done on my house, But that had nothing to do with me not going home. I didn’t want to go home because I needed more physical therapy. When DSS left, After hearing my version, no, when she left, she said “so they just sold a big old lie”) I will never go back there and I urge anyone that goes there to make sure the social workers keep up with what they’re supposed to be doing."
"I would like to acknowledge FAPA for taking care of my friend Ms. Green. She has been happy with her care here. I know everyday is not easy but you guys seem to make it happen. I hope that I ever have to a nursing home, but I do I would choose yours. Thanks for all you do."
"I am happy with my stay here at the facility. The dietary and activity department make sure they make me happy i couldn't ask to be at a better place Thanks Lamont Simuel"
