Hillview Health Center maintains a data profile characterized by higher-than-average registered nurse staffing and low staff turnover, though records indicate a 20% reduction in nursing hours on weekends and a recent increase in inspection findings.
The data indicates that Hillview Health Center maintains staffing and quality metrics that generally exceed state benchmarks. Analysis of federal records shows that residents receive approximately 4.0 hours of total nursing care per day, compared to the Tennessee average of 3.85 hours. Registered nurse staffing levels are also higher than the state average, providing 0.78 hours per resident daily. The facility demonstrates high workforce stability, with a total nursing turnover rate of 28.8% and zero registered nurse turnover, significantly lower than the state's 43.3% average for registered nurses. Furthermore, the facility reported no use of contract registered nurses or certified nursing assistants, suggesting a reliance on permanent staff which typically supports continuity of care.
However, the records reveal a notable difference in staffing between weekdays and weekends. Total nursing care hours drop from 4.0 during the week to 3.19 on weekends, a 20% reduction. Inspection history shows three health findings in the most recent cycle, including issues related to maintaining a clean environment and drug labeling, an increase from the zero findings recorded in the previous cycle. Despite these recent findings, the facility has recorded no federal fines or payment denials. Compared to its parent chain averages, this facility maintains higher ratings in health inspections and staffing levels.
The facility reported 0.78 hours of registered nurse care per resident per day, which is higher than the Tennessee state average of 0.61 hours.
Total nursing staff turnover is 28.8%, which is notably lower than the state average of 48.7%, indicating a more stable workforce.
The facility reported a 20% decrease in total nursing care hours during weekends (3.19 hours) compared to the weekday average (4.00 hours).
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"If I could leave negative stars I would. Do not allow anyone you care about to go to Hillview. Horrible patient care, nothing came of my complaints about lack of care, lack of therapy, lack of following her dietary restrictions and the horrible way some of the staff spoke to her. She was only there for 2 days when she asked me to move her based on how she was talked to and treated. Unfortunately the day before she was being transferred their negligence landed her in the hospital for several days. I hope the state shuts this place down. She was there for rehabilitation and 100% in her right mind, but could not stand or transfer on her own. The aids were downright mean to her and left her sitting in waste to the point of causing skin infections & ulcers along with a UTI and in acute kidney failure. When she told me how she was spoken to and treated it made me sick to my stomach. I was told she was being sent to the ER for "abnormal labs" and yes they were abnormal, she was in critical condition when she arrived. The ER physician and staff were appalled at the condition she was in when they examined her. The skin on her buttocks and genital area was extremely inflamed, raw, bleeding, blistered & peeling - which they documented and confirmed was directly caused by insanity conditions and negligence. My 80+ Mom told the Doctor she stopped drinking because she thought it would help her to have to urinate less. That everytime she pushed the button for someone to help her to the bedside commode or give her a bedpan she was critized and talked down to for having to go to often or the call light ignored until it was to late. Thankfully she is recovering well and getting the therapy she needed as well as being treated with kindness and dignity at the new rehab facility. I wouldn't recommend Hillview to my worst enemy. Horrible communication, I was never notified of a care plan meeting, just happened to walk in on the only one we had. When asked why I, her daughter, primary caretaker & POA wasnt notified I was told it was an oversight. When asked why she hadn't seen anyone from OT and PT had only seen her once in the 6 days she was there, when the whole purpose of her being there was for therapy they couldn't provide an answer. I could go on with several other issues I had & things I witnessed in our week there - it made me sad and angry for patients. They are severely understaffed & either under trained or just don't care. We neve met anyone from the leadership team, met the Social Worker at the 1 week in mark. Admissions was of course there day 1, eager to get thr paperwork signed. Within 2 hours at the new facility all of the leadership team came in to introduce themselves. And nursing did a full head to toe assessment, which she didnt receive at Hillview. The 1st full day we had a comprehensive care plan meeting. She received both OT & PT, head of dietary came to get her likes, dislikes, allergies, special needs, etc. The ED comes to her and all the patients rooms multiple times each week to ask how they are doing and how can he help. Hillview's staff mostly just give the vibe of not caring about the people they are caring for or having any pride in what they do, just there for the check."
"I’m writing this review to spread awareness of abuse in nursing homes.. my family sent my grandma here 3 years ago in 2022 and when we were in the hospital a nurse said this was such a great place boy was a wrong after letting my grandma stay here I remember her crying on the phone something that woman never ever did she begged us to get her out of there. Well it turned out she was being disrespected by staff and talked really down too and told us that on two or three separate occasions that she was yelled at for having to go potty and use the bathroom she would cry every single night because of the way she was treated so I am writing this to show what does happen behind closed doors at nursing homes"
"I would like to share my experience with the Hillview staff and facilities. My mother in law was a resident there 2023 through 2024. She previously was also in their rehab facility. I was very happy with the medical support staff, the physical therapists, the housekeeping staff, the activities staff and the office/social workers. She loved the different music groups that came by and the offsite trips. The medical support staff was very attentive to her needs. Hillview was recommended to me by several friends who also has family members at Hillview previously and was happy with their treatment. I would not hesitate to use their facilities for long term care or rehab for other family members or recommend them to others needing a safe and reliable facility."
"Our father was at Hillview rehab for 2 and a half months and we couldn't have ask for better care the nurses and CNA"s both night and day were wonderful. The staff treated dad and us like family. I would recommend this facility to anyone that was looking for great care for a loved one. The Vergil Cretsinger Family"
"My mother was in Rehab for 3 weeks here. Very nice place. Clean & no odor when you walk in. All the staff was very kind & helpful to my mom. Administration was very good to work with, especially Katherine & Melanie."
"The office has no idea of how to properly schedule their care givers. They do not seem to care what the clients need. Nor do does the office listen. Although they have some very good care givers, they also tend to hire people without a clue. Then lead then to thinking it's a sitting position."
"If I could give 0 stars I would. The staff does not check on patients as they should. If your loved one requires rehab, expect the absolute bare minimum. If your loved one struggles to communicate, move or potty on their own, expect they will not be checked on, turned, moved or tended to! Staff allows residents to lay in their own waste, food trays to pile up, and spend too much time chit chatting in the hallways. Also Carter County Ambulance service is allowed to block guests from leaving instead of using handicap areas or proper areas for drop offs and pickups. Whomever is supposed to be making sure staff is taking care of residents should be fired and probably half of the lazy staff. The list of infractions is long. The only thing I saw and was pleased about, was the cleanliness was top notch! Seriously! Clean! Except the dirty food trays, which probably isn't the cleaning staffs job. Just saying, if you have a choice, avoid this place! And if you love someone, please, do not let them stay here! The real shame is the people unfortunate enough to not be able to care for themselves being stuck here. It makes me weep to know what they've done to my brother!"
"Was good at first. Then they let my mom lay in bed and continue to get more sick. She ended up with sepsis, pneumonia and full blown heart failure. When I told the nurses my mom needed to go to the Emergency Department, they gave me a rough way to go. One nurse said “oh she’s just failing” They finally called and my mom went to the ER. I lost my mom 8 days later. The negligence, of the staff is why my mother passed sooner than she would have. If your loved one cannot speak much for themselves do not put them here."
"I visited my mother today. I have been an RN for 31 years and have never witnessed what I witnessed at Hillview. My mother asked for a bed pan. I would have done it myself but I had a nosebleed. The CNA came in, yelled at her and told her she was only allowed to come in patient rooms every two hours. I asked her not to talk to my mother like that and she proceeded to tell me to watch my mouth it was none of my business. When she left my mom who is 80 and totally in her right mind started crying and saying that this CNA and one other had talked to her like this for the week she had been there but she didn't want to cause trouble. And to top it off the therapist didn't know my mom's diagnosis and was not allowing her to do weight bearing has tolerated as the surgeon had said. Anywhere I have ever worked if therapy had a doubt about weight bearing status they called to clarify. I am moving my mom out tomorrow. I would not send my dog or my ex there."
"Great place! Hillview has an excellent program, it’s safe and they have a wonderful, friendly, and caring staff. The facility is very attractive inside and out and they do a really nice job of decorating for the holidays. This being in mid-October they have decorated for the colorful Fall season with several special displays for Halloween. In sum, Hillview is a patient centered health facility, professional in every way, and is blessed with a highly experienced and talented staff."
