Records show a facility with a declining overall rating, characterized by health inspection findings that are nearly triple the state average and lower-than-average registered nurse staffing levels.
Data from the Centers for Medicare & Medicaid Services indicates a significant decline in quality metrics at this facility over the past 24 months. The overall rating dropped from a three-star to a one-star level, primarily driven by a sharp increase in health inspection findings, which rose from 20 to 43 in the most recent cycle. These findings include failures to timely report suspected abuse and neglect, failures to maintain medical record privacy, and inadequate supervision to prevent accidents.
Staffing levels show specific areas of concern relative to regional benchmarks. While the total nursing care of 4.01 hours per resident per day is within range of the chain's average, the amount of care provided specifically by registered nurses—who have the highest level of training—is 0.43 hours, significantly lower than the state average of 0.65. Furthermore, records show a drop in weekend staffing to 3.78 hours, suggesting residents receive less direct care on Saturdays and Sundays. Although the facility does not use contract or agency staff, the total nursing staff turnover rate of 43.5% is higher than the California average of 38.1%, indicating more frequent changes in the caregiving team. Short-stay quality measures also lag at a two-star rating, while long-stay measures remain a relative area of clinical stability at four stars.
The facility recorded 43 health inspection findings in the most recent cycle, which is nearly triple the California state average of 15.6 findings per cycle.
Registered nurse care is 0.43 hours per resident per day, which falls below the California state average of 0.65 hours.
There are 22 uncorrected fire safety findings, including issues with automatic sprinkler systems and portable fire extinguishers.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"SO rehab was a fantastic facility. The priority is care and they were phenomenal with my mother-in-law. My MIL had impaired mental capacity due to an UTI but they made sure she was monitored and provided her with the best of care possible."
"PLEASE READ this before considering sending any loved one to this rehab. I wouldn't send a dead dog to be in the care of certain members of this staff at Sherman Oaks Rehab who worked in the evening at the time of the incident. While some of the day staff was quite kind and caring, my parent had experiences so psychologically traumatizing and frightening with members of the night staff that to this day she is afraid to go to another rehab. She called us begging to leave. She had one member who reportedly would just come in and stare at. her. She felt her medical conditions which can become life threatening and result in a stroke if not stayed on top of were neglected and her access to medications was not handled in any kind of a timely manner. She felt so vulnerable and helpless followed her surgery and she had every right to expect the staff there would be kind, compassionate, and respectful. When loved ones -one who is a working professional in healthcare herself-came to pick up my parent they reported patients standing in the doorways in their gown begging for water and other assistance, one of whom reportedly had been waiting so long that she decided to bring her water herself. My parent has repeatedly said never in her life has she had such an experience, even with various hospital and rehab stays at her older age, and to this day, she is haunted by her terrible experience at Sherman Oaks Rehab. Similarly, I was as well by the cruel indifference I experienced by the staff when I was on the phone with them trying to sort out what was happening. They were extremely indifferent. Again, if you love your loved one, I would look at other rehabs before this one. I was appalled at the way in which she was treated. Never"
"Marissa answered my call and was extremely polite. Looking forward to working together."
"BEWARE OF SHERMAN OAKS HEALTH AND REHAB! I suppose OK for healing bones and PT, BUT NOT a place for sick people. My husband was there for about two weeks+ and got progressively worse, including his delirium. The site doctor whose care he was supposed to be under never saw him, NOT ONCE, and refused to call me. When I complained, a new site doc was assigned; he visited once and did not examine my husband. My husband was losing control of swallowing and was never provided an appropriate diet, even after I requested it. I later discovered he had developed aspiration pneumonia WHICH WAS NEVER ADDRESSED. When I insisted that he be transferred to a hospital, the nurse said she would consult the doctor. Consult?? When I threatened to call 911, doctor authorized transport. When we arrived at ER, I was told that he was VERY sick and that he would have died had he stayed there. Here’s another kicker, Fortunately, we would not have stayed anyway, but Sherman Oaks Health and Rehab mislead Medicare about my husband’s ability to function and Medicare denied further stay!! He was delirious, could not walk, could not talk coherently, could not eat and died in the hospital 3 weeks later. BEWARE! P.S. Public areas ok, but the 2 patient rooms we occupied looked like something out of a flophouse."
"Our 90 year old Mom has been at Sherman Oaks Health & Rehab for 9 years. The staff are so wonderful kind & compassionate. From the 1st Day to present Erma, Rachel, Juan, Tyrone, Belle & so many others have given our Mom the best care possible. Steve Haas is the amazing director here & he truly cares. It shows in how well this facility is run. The facility is immaculate & bright. We feel so fortunate to have our Mom here where everyone is professional & so caring."
"My father is in this facility for already eight month under a very good care of these hard working people. There is no other way to describe them, but angels. Each and every one of them do their hard work with passion and love. I want to personally thank Oleg Ashkenazi (physical therapy), Olga Senchea (occupational therapist), Simone Davenport (lvn nurse), Clair (activities), Irma Rivera (CNA) and each and everyone there whose hard work makes such a difference in the lives of these elderly patients."
"I felt I must write about , Sherman Oaks Health and Rehab. My best friend has been there for 2 1/2 years , the quality of his care is excellent. The nursing staff is wonderful especially, the C.N.A.'s who do the hard work, treat the patients with such kindness and love.He has a Dr, who comes into see him 3 times a week , and is excellent, for most nursing homes, this is unheard of. The place is emaculate and the residents are always well dressed and groomed. The last thing that I would like to say is they have wonderful activities for the residents, once a month they have a candle light dinner. With small tables, white linen , candles, the same as a good restaurant. So I leave my friend in the best of hands, with no worries."
"My mother was at Sherman Oaks Health & Rehab for approx. 4 years, and I thank God for this wonderful place and the wonderful people there! My mom had suffered a stroke 6 years ago, and was in two other nursing facilities that were horrible Then we learned about Sherman Oaks Health and we were fortunate to get my mother in, where she was attentively and lovingly looked after in every way. My mom finally succumbed to her stroke, but would have passed away years earlier without the attentive care of the staff and people here. God bless Sherman Oaks Health & Rehab!"
"My mom was there for simple monitoring 2 years ago after "routine" closing off an upper GI bleed. They had her there for 2 weeks constantly unable to hold anything down. They should have sent her back to the hospital but they didn't. I had no clue. I thought they knew what they were doing. They finally send her back and they redo the procedure. then back to this place again to regain her strength. My mom was always a picky eater and refused to eat it. She lost weight she didn't need to lose. She was sent back to the hospital with anemia. I refused to send her back to rehab. Unfortunately, by then it was too late. Mom was at the beginning of Alzheimer/s and was 80 years old, the disease skipped a few stages and now she has to live in one of these places but never this one. My mom's stay at this place ruined her life. Sadly most of these places are like this."
"I have never seen anything more disgusting, smelly, old and nasty. My mom was suposed to stay there for few days for rehab purposes, after an hour she called, begging me to pick her up. I don't wish anyone to end up in a place like this. I had PTSD from the experience."
