Henderson Health and Rehabilitation maintains high staff stability with zero agency use, though its health inspection findings and daily nursing hours remain below state averages.
The data indicates a facility with a complex quality profile characterized by strong staffing stability but lower-than-average health inspection performance. Analysis of staffing records shows a total nursing turnover rate of 38.2% and a registered nurse turnover rate of 31.4%, both of which are significantly lower than the Nevada state averages of 46.3% and 47.1% respectively. Furthermore, the facility reported no use of contract or agency staffing, suggesting a high level of staff continuity. However, total nursing care hours per resident per day (3.45) remain below the state average (4.34).
Inspection records show a persistent challenge with health standards, as the facility received 13 findings in the most recent cycle and 12 in the preceding cycle, both of which exceed the state average of 9.6. These findings included issues related to infection control and the governing body's management policies. While quality measures for long-stay residents are rated at the highest level by federal regulators, measures for short-stay residents are rated lower. A financial penalty of $40,830 was issued in December 2023. Compared to other facilities in the same chain, this location carries a lower health inspection rating but higher staffing scores.
The facility reported zero percent use of contract or agency staffing, indicating all care is provided by direct employees, which is significantly lower than average for this region.
Weekday to weekend nursing care levels show a drop of approximately 11%, falling from 3.45 to 3.06 hours per resident per day on Saturdays and Sundays.
Records show 13 health inspection findings in the most recent cycle, including failures to follow infection control programs and food storage standards.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"I rating this facility a ZERO but it only allows a ONE star. My mother was sent here due to her qualifications per insurance , I preferably would have liked her to be placed anywhere else but here! -Lack of courtesy (just there for paycheck). -horrible food and cold : Corn dog or hotdogs with fries etc the list goes on. Breakfast was two hours late floppy and cold meal. I had to go shopping for her food so she can have healthier meals to eat. She walks to the vending machine that’s available to avoid food presented to patients. She asked for coffee but they didn’t even have sugar (wow!) -We requested clean beddings and was told they were out of sheets at the moment. -Room floors needed sweeping/mop which they only did it because we asked. -Facility has an unpleasant smell that you can still smell through face mask. -RN messed up on my mom’s IV and meds was wasted which kept her to stay an extra day. NEVER seen or able to communicate with an actual Doctor. -A guy came in as an X-ray tech smelled like cigarettes and in street clothes not professional at all. -My mom needed a shower after coming from UMC it been 5days. I asked for assistance and was told it wasn’t her day to shower. After explaining that my mom still had blood in her hair from a wound, if she can get a shower, so they squeezed her in (Thank you for that). My mom was allowed to shower herself because she is still very well capable. -We was told the facility been upgraded BUT honestly it’s a mess, it smells and needs alot of improvements!! I really do not like to leave bad reviews but this needed to be mentioned. Today she is supposed to be released as I write this I truly hope I can get her out of this place TODAY! On a good note the Physical Therapy was the ONLY GOOD THING that was provided for my mom. I wish I knew the PT name which I met during session- She was great! And my mom said that’s the only thing she was happy with!"
"If I could give negative all the stars in the universe I would. I sent my semi able bodied mother there for a 5 day respite. The day she left she got up showered, had conversations with the family and assisted in getting onto the gurney out. I had to send her due to an emergency. When She got home just 5 days later and was vacant and void, uncommunicative and passed 6 days later. If you LOVE your parent, significant other - whomever. DO NOT SEND THEM HERE. I miss you ma, so much💔"
"Not Happy!!! Zero stars need to be added. Horrible treatment & care! Most staff doesn't care or must be terribly understaffed! One Flew Over the Coocoos Nest seemed more tolerable. Sad conditions. People going without food if they can't eat what is served, they're sick of hotdogs and tiny sandwiches with hardly anything on them. Women walking around without proper clothes, bras or anything. Men in diapers that need changing. It has driven patients mentality down and taking what dignity they had left away. Letting patients leave the facility looking like homeless degenerate people. Not caring about some patients going in others rooms, food on floors, phones not answered, promises not kept...it goes on & on!!! I CANNOT STRESS ENOUGH TO BEWARE IF YOU CARE ABOUT WHO IS BEING PLACED HERE!!!!!!! Seems to be just a money maker not a care facility sadly. God help those in there & the extremely few staff members that do care 🙏"
"My father is in a shared room where his roommate listens to the TV so loud we can not even have a conversation with him on the phone. I left a voicemail with his case worker and never received a call back. My dad is literally losing his mind in this facility. This place should be shut down. Deplorable."
"Nurses don't know how treat patients Most CNAs don't know how to treat patients. Staff are rude, mistreat patients. You are told you are a liar when you complain. Call lights are not answered for over an hour. Food is brought to you cold and over an hour after then it should of been brought or and hour earlier."
"The CNAs are wonderful. The Nurses are very knowledgeable and nice. The Doctors are second to none. The food has improved. The rehab is incredible here. This place is kept clean by house-cleaning staff, everyday. Overall, operations are phenomenal. This is a very smooth running place, especially for it's size. The reason I gave a four star rating instead of a five star rating is because the place lost a very valuable item (my clippers) when I went to the hospital, and then the administration tried to cover it up. I can prove my assertions, and I hope the owner of this facility will look into this. This makes me afraid to go to the hospital, what will come up missing next? Maybe my laptop?"
"If 0 stars were an option, that would have been my choice. The majority of the nurses and CNA's do not care about the patients care ( there were only about 3-4 of staff that actually took time for my mother's care) . The response time from the time the patient presses the nurses button is 30-60 minutes. There should be an investigation done on this facility. After we got the Director of Nursing involved, my mom was scheduled to be discharged. Make sure if your loved one goes here to do an inventory of all belongings as they lost my moms dentures from day one and will not have them before her discharge date. The rehabilitation team is good."
"Stay as far away as possible! I’ve been here 48 hours and the way the staff treats their patients are so unprofessional. The day shift people and therapists are great but once the sun goes down it’s a whole different world. Tying people to chairs with bed sheets, teasing and making fun of them about their appearances. A lot of these patients have dementia and they're teased about carrying babies. We chose this place because the ratings were good and the place looked clean. Definitely not the case. I won’t allow my family come visit as this is more of an insane asylum than rehab. My family and I plan on running out of here STAT. Don’t believe everthing you read/see on the internet. This place is scary! And don’t get me started on the food! Gross!"
"This is not a rehabilitation facility It's more of a mental state hospital. They're understaffed and no one wants to do someone else's job so you are there helpless. They were supposed to set me up with home health care when I left their facility and a wheelchair and I don't have either. They failed to tell me that home health care did not accept my insurance and they did not provide me with another home health care or physical therapy. I would not recommend this place especially if you need physical therapy. If you don't mind people screaming all night long and people walking up and down the hallway banging the walls and yelling this might be the place for you. But if you're trying to recover from surgery it's not peaceful and restful. Very disappointed in this place"
"The staff were compassionate, knowledgeable, and attentive, making my family member comfortable and well cared for throughout their time there. The facility itself is clean, welcoming, and equipped with everything needed for effective rehabilitation. The therapists are incredibly skilled and tailored to the treatment plan to the specific needs, ensuring steady progress. They truly go above and beyond to support recovery and wellness. I highly recommend this center to anyone seeking a professional and supportive environment for their health and rehabilitation journey!"
