    Summary

Frasier Meadows Health Care Center is a non-profit facility characterized by staffing levels and staff retention rates significantly above state averages, though its record is also marked by recent high-severity inspection findings and financial penalties.

    Detailed Analysis

The data indicates that Frasier Meadows Health Care Center maintains staffing levels and stability metrics that exceed state averages. Records show 4.83 total hours of nursing care per resident per day, including 1.05 hours from registered nurses, compared to state averages of 3.70 and 0.83 respectively. The facility demonstrates high staff retention, with a nursing turnover rate of 24.1%, which is roughly half the Colorado average. Furthermore, the drop in nursing care during weekends is limited to 7.4%, suggesting consistent coverage throughout the week. While the facility uses contract staffing for 15.3% of its registered nurse hours, its overall reliance on agency staff remains low at 6.9%.

However, the inspection record shows recent high-severity findings. Specifically, a 2024 inspection cited the facility for issues regarding the prevention and treatment of pressure ulcers, categorized at a severity level indicating actual harm to a resident. This contributed to one of three fines issued since late 2023, totaling $49,202. While the total number of inspection findings per cycle (1.0 to 2.0) is lower than the state average of 8.7, the severity of recent individual citations remains a significant data point in the facility's profile. Analysis of fire safety records shows 21 uncorrected findings, including issues with sprinkler system maintenance and fire alarm testing, which represents a higher volume of findings than the state average of 6.2 per cycle.

    Key Findings

The facility reported 4.83 total hours of nursing care per resident per day, which is 30% higher than the Colorado state average of 3.70 hours.

Staff stability is reflected in a 24.1% nursing turnover rate, significantly lower than the state average of 48.6%, and zero administrator departures in the last year.

Recent inspections identified a high-severity finding related to pressure ulcer prevention and care, resulting in a fine of $21,678 in April 2024.

    Performance Trends (24 months)

This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

    Staffing Deep Dive

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"Beautiful facility! I was here for a conference, and both the staff and residents were really friendly. The food was good, and everything was very hospitable. The photo is what you see when you walk out the front doors. Bonus tip: Pay attention to the art on the walls. One of the residents told me the artwork is on loan from the Denver Art Museum and rotates seasonally."

"⭐️⭐️⭐️⭐️⭐️ Exceptional Care at Frasier Meadows! Our experience with Frasier Meadows has been nothing short of exceptional. From the moment we walked through the doors, we knew we had found the perfect place for our loved one. The staff is not only highly skilled but also incredibly compassionate and attentive, always going above and beyond to ensure the comfort and well-being of every resident. The facility itself is clean, welcoming, and well-maintained, providing a warm and safe environment. The medical care provided is top-notch, with a team that is always available to address any concerns and provide personalized care plans. The variety of activities and social events offered also ensure that residents are engaged and connected, making it a true community. We had complete peace of mind knowing that our loved one was in such capable and caring hands. Frasier Meadows truly deserves its five-star rating, and we highly recommend it to anyone seeking the best possible care for their loved ones."

"A very caring, comfortable, and well staffed facility. The complex is beautiful, and the living quarters are cozy, clean apartments. There is an abundance of activities for the residents to partake in as well. Frasier Meadows took very good care of my great Aunt and Uncle until their passing. They also hosted a beautiful memorial service for both."

"I own a business named Colorado Wheelchair. Today I had to do an evaluation for one of their residence power wheelchairs. I cannot begin to tell the public how impressed I was with their protocol on safety and sanitation during this pandemic. It was over the top and in my opinion absolutely 100% appropriate. I have been in many facilities since the outbreak and none of them even comes close to the level of necessary caution that Fraiser Meadows exercises. Super impressed! I have worked with the staff and residents at this location for over a decade. From their independent and assisted, to skilled care, this is the finest senior facility in the entire state. Their care on a clinical level (Nursing, P.T., CNA) and a personal level (meeting the daily and moment by moment needs of their resident's) is matched by none."

"Nice place. I love the patients. CNAs are lazy, not helpful could not be bothered, most barely if at all speak english Nurses are for the most part nice but overworked because they are having to do CNA duties too. Patient per RN ratio is a little less to be desired but where is there a home where they have enough staff. Food is good. Clean nice place. Lots of activities, pool, spa, beauty shop, gym saunas, dancing nice park across from facility beautiful views of mountains, quiet neighborhoods aroynd. Church across street from facilities. Families can come and go at all hours if need be. Everytime I go in there is a front desk person who is always friendly. If I had too put my mum in a home Boulder this would be my top 1 out of 2."

"Went here to do some service with my daughter. Based on what I saw there and the comments of the residents, this is a really nice facility."
