NHC Healthcare, Bristol maintains high clinical quality scores and stable leadership, but exhibits high nursing staff turnover and recent high-severity inspection findings involving resident safety.
Data from the Centers for Medicare and Medicaid Services indicates a complex performance profile for this facility. The facility maintains high marks in quality measures, which track clinical outcomes for both long-stay and short-stay residents. However, staffing data reveals significant volatility. Total nursing staff turnover is recorded at 60.2%, meaning more than half of the nursing workforce left within a 12-month period. This is considerably higher than the Virginia state average of 48.4%. Additionally, there is a measurable decrease in care during weekends; hours of nursing care per resident per day drop from a weekday average of 3.89 hours to 3.50 hours on Saturdays and Sundays.
Inspection records show a mix of historical consistency and recent high-severity findings. While the facility has averaged fewer inspection findings per cycle (3 to 5) than the state average of 14.4, a 2025 report documented an accident-related finding categorized as 'Actual Harm.' This incident resulted in a federal fine of $12,735. Despite these challenges, the facility demonstrates high stability in its leadership with zero administrator departures and maintains a workforce entirely composed of direct employees rather than temporary agency staff, which often supports better continuity of care. Compared to its parent chain, National Healthcare Corporation, this facility exceeds the chain's average health inspection and quality measure ratings.
The facility reported zero percent use of contract or agency staff, indicating all nursing care is provided by direct employees, which is significantly lower than many state peers.
Public records document a recent inspection finding involving a failure to prevent an accident, resulting in a severity level of 'Actual Harm' to a resident in August 2025.
Annual nursing staff turnover is 60.2%, which is approximately 12 points higher than the Virginia state average of 48.4%.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"My uncle was a resident at NHC Bristol for an extended time, sadly he passed away. My family wishes to thank the entire staff for their professional care from the start of his illness to the end of his life. Everyone was very friendly and helpful, not only to him but to our entire family. The building was clean and looked nice. The food was unexpectedly great, it was not the standard institutional food that I had experienced at previous hospitals and care facilities. Whenever we had questions or needed anything, we were always helped by a smiling staff member. Through all of this the memory we will hold forever was at the very end, as his time grew short the love and compassion that filled his room as he was surrounded by those who had cared for him, his angels, comforting the family and watching over him. The staff has my utmost respect for the job that they do. I highly recommend this place if you are considering options for your relative who needs this type of care."
"My father was a patient at this facility in 2016 before he passed away. I cannot day enough about how caring and knowledgeable the staff was. They made his final days as comfortable as possible. The Administrator, Natalie, and Social Services Director, Sarah, set up private accommodations so family could have private time with him. One particular CNA, Juan, and nurses Sue and Shea, always kept me informed and treated us all with kindness and respect. I would recommend this facility to anyone... in fact, I wouldn’t have my family anywhere else."
"My grandmother had open heart surgery and went to NHC for 2 weeks of rehab. The first night she pressed the button for someone to help her go to the bathroom and a tall, slim, man with dark hair and dark beard came, turned the beeping off and left. He never came back to help her. She had to get up and risk falling after surgery to go to the bathroom. She pressed the button again a couple hours later and no one ever came, forcing her to go again by herself. She was not given any pain medication and laid there in absolute agony for close to 24 hours. When she told one of the caretakers that she would be going home the following morning, she tried to intimidate her into staying by saying that her insurance wouldn’t cover it and that she would not be able to take her medication home with her. She had such a horrific experience that she didn’t care. She has been home with no medication all weekend because of the abysmal “care” she received here. Her roommate had to yell “help she’s fallen” in order to get the employees’ attention enough for them to bother coming by. Please do not risk sending your loved ones, or yourself, here."
"I am so impressed with this center! I have been in many rehab and nursing facilities as a visitor over the years with some good and some bad experiences. When the time came for my relative to need assistance in healing I knew immediately that NHC was the only place for him. The staff is very professional, kind and patient. The food is superior, the center is clean (smells clean too) and the therapy staff is excellent. I highly recommend NHC in Bristol."
"My mother-in-law was at NHC in February of this year. With COVID restrictions we were not allowed to visit at first. We were allowed to call and check on her anytime day or night. She was treated like family. The Doctors, Nurses, Therapists, Dietary staff and everyone else that was involved in her care showed such compassion. They were completely open and honest with us about everything. They are wonderful and caring folks."
"Our 94 year old father was at the facility from January 2017 until this July when he passed away. All the staff on the 2nd floor and Melissa the PA were wonderful during the last days of his life. They not only cared for our father but they cared for the family. Between my brother and I we were there 5 / 6 days a week and at all different times. Not to mention other family members and friends that were in and out. So we saw the care that was given to not just our loved one but the other patients. I will not say that we never had any issues in the year and half that he was there, but what I will say is that when we did and we discussed it with the nursing staff, housekeeping or laundry it was always resolved and handled in a professional manner. Natalie the Administrator was also available to assist if needed. All the staff from administration to the maintenance department treated our father with love and respect. He was happy there and we had comfort knowing he was being cared for. And he would tell us if he wasn't being treated right. He did not hold back... :) No facility or business is perfect and never will be but the important thing is how they respond when there are issues and if they are corrected. That is what we found at NHC Healthcare Bristol. I have recommended NHC to many people and will continue to do so."
"My grandmother was at NHC after she had open heart surgery . The nursing staff gave her wonderful care and was attentive and therapy helped her so much. Also, It is very clear that Michelle, the director over nursing cares deeply for the patients to ensure they are well taken care of."
"The rehab was like a bunch of chickens running around with there heads cut off. The room my mother was in was too small for 2 people. Mo comfortable chairs for family members to sit on. An my mom is on oxygen 24/7....but they didn't even have a long enuff oxygen hose for her too get back & forth too bathroom. They would put her in an wheelchair with a oxygen bottle too get to bathroom. But i will say the nurses were very nice & helpful. An the food was great, i would go every evening and have supper with my mom."
"My mother has been at NHC Bristol for four months. From mom's first day of arrival, the care received has been above and beyond what I have experienced both as a daughter of an aging parent and a Geriatric Care Manager in healthcare and geriatrics. Upon mom's arrival, I met with the management team to discuss her care plan and my concerns as I live out of state. Immediately I was impressed with the number of staff that met with me and, most importantly, listened as I provided information crucial to her care, particularly her medical and mental health. As my mother has several complex chronic diagnoses, the quality of medical care and the way it is delivered is first and foremost to her family. Through moms' good days and bad days, the staff, including the Administrator, Director of Nursing, physicians, nurse practitioners, nurses, CNAs', dietary, therapy, laundry, housekeeping, and maintenance, have been accessible, invaluable, professional, and personable. Activities have played an essential aspect of socialization for mom as she is social but often needs to be encouraged to attend events. As a result, mom participates in more activities longer, primarily due to the activity department's patience and gentle persistence. In addition, the administrative staff has been essential in assisting me with obtaining insurance that best meets mom's needs as a resident and if she returns to the community. The team has always been accessible to me, day or night, for any and every concern or question I have. There has not been one time that I have called and not received a return call. For anyone who has experienced the anxiety and stress of caring for an aging parent, knowing that your parent is in a safe place is essential. A family needs to know that their loved one resides in a facility where quality care abounds, concerns are addressed, dignity is respected, and questions get answered. NHC Bristol excels in every area. I know that at NHC Bristol, mom is safe and taken care of, and with the progression of her illnesses, you are taking care of her better than we could do. Thank you for all you have done and continue to do, and most importantly, thank you for providing my family and me the peace of mind that mom is in great hands!!!"
"My Mom was transferred to NHC today from Bristol Regional for rehab. She was in Bristol Regional for 3 weeks due to a heart attack and barely able to breathe. Mom has heart failure and stage 4 COPD. She is on oxygen 24 hours a day, takes several medications and requires breathing treatments several times a day. This evening Mom calls the nurse for a breathing treatment. Her response was “we don’t do breathing treatments here”. Mom explained that she would not be able to breathe without it. The nurse comes back with a breathing treatment of albuterol which is not what Mom takes but he used it anyway. This nurse lied. No breathing treatments, how was she able to get one? The hospital sent over 2 new antibiotics as well as Mom’s regular medications. Mom asked for them and was told that they couldn’t give them to her until a doctor examines her the next day. Mom explained how these were necessary. I got a call from Mom really scared at midnight to come get her. She knew she wouldn’t make it. The nurse asked if I was taking Mom AMA. I told her yes, Mom would die overnight. This is not a rehab. It is a place for patients to stay until they die. My family was really hoping that Mom would get the treatment she needed here. I gave the rating as 1 to enter this information. They deserve a 0. Someone with authority needs to look into this place. They need to be shut down!!!!"
