Red Cliffs Post Acute is characterized by a federal abuse icon, high staff turnover rates, and nursing care hours that fall below state averages.
Data from the Centers for Medicare and Medicaid Services (CMS) reveals significant disparities between this facility and regional benchmarks. The facility currently holds a 1-star overall rating. Staffing levels are a primary area of concern; the facility provides 3.03 total hours of nursing care per resident per day, which is approximately 18% lower than the Colorado state average of 3.70 hours. Furthermore, registered nurse staffing of 0.51 hours per resident per day is notably below the state average of 0.82 hours. The facility also exhibits a high nursing staff turnover rate of 61.0%, indicating that a majority of the nursing staff has changed within the last year.
The inspection history shows 13 health-related findings in the most recent cycle, compared to a state average of 8.7. These findings include failures to maintain an environment free from accident hazards and issues regarding resident self-determination and choice. While the facility maintains a high 5-star rating in long-stay quality measures, its 1-star staffing rating and 2-star health inspection rating contribute to its overall profile. Public records also show the presence of a federal abuse icon, signaling that regulators have identified specific incidents related to resident safety or abuse. On a positive note, the facility has recorded zero federal fines or payment denials in the most recent reporting period.
The facility has been assigned a Red Abuse Icon by federal regulators, indicating a history of abuse or neglect citations.
Total nursing staff turnover is 61%, which is higher than the Colorado state average of 48.6%.
Weekend staffing levels show a decrease in registered nurse availability, dropping from a weekday average of 0.51 hours to 0.21 hours per resident day.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.
