    NursingHomeIQ Analysis

    Summary

The records indicate an overall quality rating of one star, characterized by high nursing staff turnover, significant health inspection findings, and staffing levels that fall below Nebraska state averages.

    Detailed Analysis

Data from the Centers for Medicare and Medicaid Services (CMS) shows the facility maintains a one-out-of-five star overall rating. The staffing profile indicates 3.43 total hours of nursing care per resident per day, with registered nurse care specifically at 0.31 hours per resident per day. This registered nurse staffing level is less than half of the Nebraska state average of 0.71 hours. Furthermore, total nursing staff turnover is recorded at 74.1%, and registered nurse turnover at 78.6%, indicating that more than seven out of ten nursing staff members left the facility within a twelve-month period. Weekend staffing levels show a 13.6% decrease in total nursing care hours compared to weekday averages.

Inspection records reflect 19 health inspection findings in the most recent cycle, including 19 related to complaints. While this is an improvement from the 31 findings in the previous cycle, the frequency remains high compared to the state average of 6.7. Notably, a severity level 'G' finding was recorded for a significant medication error, and a severity level 'J' finding—the second-highest level of concern—was also documented. Fire safety records show 30 uncorrected findings, including issues with smoke barrier doors and construction materials. The facility is part of a chain with an average overall rating of 1.5 out of five stars, suggesting consistent data patterns across its affiliated locations.

    Key Findings

The facility reported 3.43 total hours of nursing care per resident per day, which is 17.7% lower than the Nebraska state average of 4.17 hours.

Records show a total nursing staff turnover rate of 74.1%, significantly exceeding the state average of 49.3%.

Federal regulators issued three penalties over the last two years, including two fines totaling $39,481 and one three-day denial of payment.

"We had a wonderful experience with Emerald Mercy. The nursing staff took exceptional care of my grandma during her stay. They were not only attentive and professional but also incredibly kind and compassionate. It gave our family real peace of mind knowing she was in such good hands. Every time we visited, she spoke highly of how well she was treated. We're so grateful for the support and care she received—thank you to the entire team!"

"During multiple visits, I witnessed and overheard nursing staff swearing at one another, playing music loudly on personal phones, and ignoring ringing phones that were within arm’s reach. These behaviors create an unsafe, disrespectful, and unprofessional environment and are inconsistent with accepted standards of care in a skilled nursing facility. Residents depend on staff to meet their most basic needs, and this lack of responsiveness places resident safety and well-being at risk. I also observed what appeared to be a leadership staff member yelling private patient information from her office on the third floor to employees seated at the nurses’ station, an area accessible to other staff, patients, and visitors. This conduct represents a serious breach of patient privacy and confidentiality and may constitute a violation of HIPAA regulations and resident rights related to dignity and privacy. My loved one was admitted to this facility for care and support; instead, he was met with dismissive attitudes and resistance when requesting assistance. I personally observed staff arguing over who should respond to call lights, and on multiple occasions, those call lights went unanswered. Failure to respond to call lights is a direct violation of resident care standards and poses a risk to resident safety. I asked about light history/tracking data they do not have this in place. Most facilities have a tracking system in place so they know when call lights are or are not responded to. Basic care needs, including providing clean linens and ensuring water pitchers were filled, were routinely neglected. These failures represent a lack of adherence to fundamental nursing care requirements and resident rights to adequate hydration, hygiene, comfort, and dignity. I would NOT RECOMMEND this facility to anyone. Do your research, I am sure this review will be taken down once it is discovered."

"This place is absolutely appalling. My mom had pancreatic cancer and was here for rehab. Her health deteriorated rapidly and it was clear she was no longer able to do rehab or go home but the staff kept treating her like she was supposed to be doing everything by herself. Much of the time I was there to visit her I spent picking up her room, cleaning toilets, and cleaning up the bucket she threw up in and trying to help her sit up or get out of bed. We’d hit the light for help and it would be 15-30 minutes before anyone showed up and then they would turn the light off and say they needed to get someone else to do it. The day she arrived they gave my mom a broken wheel chair and said they’d get a new one, we asked everyday and was told “we’ll get it”, we never got one. We asked for a bedside commode and never got one until the last night she was there and fell. One evening I left for about 90 minutes and when I came back my mom was crying because a nurse argued with her after she’d been hitting the call light and yelling for help for close to an hour because she was in pain. That guy must have felt guilty too because whenever he saw me he ran off and hid. Most of the staff are either on their phones or in the hallways complying about the residents, their job duties, or arguing with each other. Whenever we did get someone to help they usually sighed or rolled their eyes and made comments like “we’re not really supposed to be doing this” like treating a dying woman with some compassion was too much for them. I hate this place, I hate the fact that some of my mother’s last days she was upset and scared. Thank god I was able to get her moved before she passed. The assistant nurse manager, social services director and one other nurse (I didn’t catch her name) are nice and kind people, they deserve to work somewhere better. Everyone else, you can’t fire them fast enough. Every official I can file a complaint with about this place will hear it. I’ll keep checking this review to make sure it never comes down, if it does I’ll keep reposting it."

"The rooms were clean. The staff is polite and attentive. They took him to physical therapy every day."

"My mom just recently had a stay for rehab for a broken hip. The first time walking in I was so impressed with how inviting the lobby was, bright and clean! The nurses treated her like family as well as our big group that were always coming and going! Very happy in the decision to go to Emerald, didn’t have to worry about my mom being taken care of!! Thank you all!!!"

"I have a relative staying at the Mercy location. They have taken very poor care of her. They are not mind and the room is left dirty as is her body. She is left soiled for long periods and this has injured her skin. The nurses say they can't get staff to do their jobs. I am sad she is there and can not recommend the facility."

    Lakrysta12/23/2025

"They treat my grandma horribly and neglect her. She has also been stolen from many times. They don’t take her to check up appointments for her diabetes and don’t check her blood sugar. She has cirrhosis of the liver and bad kidneys and she is required to get check ups for those too. She was supposed to get an appointment for her hearing months ago and they never took her."

"**update** They keep reporting my review as fraud because they dont want you to know the truth!! My mother has never been this sick her entire life. She is not improving and I'm getting a lawyer! If I could give this place 0 stars I would. If you love your family member, DO NOT put them here! The first violation was 2 days after my mother got there was them giving her critical blood pressure meds 2 hours late then 4 hours late for a requested Tylenol. My mother has stage 3 lung cancer and was only there for rehab. The next violation was she had only been given 1 shower in the week and a half she had been there. Yesterday, they left her restrained in her regular chair without her call button and walker to get herself to the bathroom! Today I stopped in and she was completely lethargic, sitting up in a desk type chair complaining of extreme pain in her bottom. All of the staff were sitting at the nurse's station on their phones! My mom developed swelling in her right ankle that was left unaddressed when her pulmonary doctor said it has to be reported IMMEDIATELY! I am in health care and have been for many years. This place is unable to provide BASIC NEEDS! I demanded my mom be sent to the hospital and moved her out. This place shouldn't be open. **Update ** my mom was in respiratory failure and they did NOTHING to help her! She was absolutely fine a week ago when she got there! Another day and my mom could have died!! She's still in the hospital because of their neglect and it's been over a week!"

"My mom has been here for a couple of years and I almost took her out until 7 months ago. This place has changed for the better! It's not only beautiful but the admin team is great. Very friendly and helpful. They have a med aide names ET who my family absolutely loves, keep her because she is great!!"

"My father in law wasn't even here for 24 hours before he was rushed to the ER due to the staff's incompetence and arrogance. The only reason this place is even open has to be because the state of Nebraska doesn't have the funding or manpower to investigate and shut down. My father in law was transferred here from Immanuel hospital for rehab. He has Type 1 Diabetes and the nurse mismanaged it so badly, he nearly ended up in a diabetic coma. He was in need of insulin, testing over 400 and complaining of double vision and vomiting, but the nurse continued to insist to him and my mother in law on the phone that he "just needs to eat because he is diabetic". Unbelievable audacity to argue (incorrectly) with the family that has been managing this for him for years. We were fortunate enough that his daughter was able to rush down and call an ambulance where he was sent to Bergan ER, where he was put on a drip and told they get people rushed in to their ER from Emerald FREQUENTLY! Heartbreaking he had to go through this for no reason."
