Records show a facility with low overall quality ratings, high nursing staff turnover, and significant financial penalties resulting from inspection findings that documented actual resident harm.
Data from the Centers for Medicare and Medicaid Services (CMS) indicates significant performance challenges at this facility across several evaluated domains. The facility maintains a one-star overall rating, which is lower than the Florida state average of 3.2 stars. Public records show a high frequency of inspection findings, with 15 cited in the most recent cycle and 22 in the previous cycle. Notably, four of these findings were classified at a severity level indicating actual harm to residents. The facility has also incurred substantial financial penalties, with two fines totaling $312,600 issued within a ten-month period, significantly exceeding the state average fine amount of $30,917.
Staffing records reflect high levels of instability. The total nursing staff turnover rate increased from 76.6% to 84.5% during the tracking period, which is double the Florida state average turnover rate of 42.3%. High turnover typically suggests that residents may not receive care from the same staff members consistently. While the total hours of nursing care per resident per day (3.73) is near the state average of 3.85, the lack of reported specific data for registered nurses and licensed practical nurses in recent periods prevents a detailed analysis of the clinical staffing mix. Additionally, the facility's parent chain, Blue Ridge Healthcare, holds a one-star average overall rating across its locations, aligning with the individual performance metrics of this center.
The facility was cited for failing to provide care and assistance with activities of daily living for residents unable to perform them independently.
Two federal fines totaling $312,600 were issued between November 2023 and August 2024, which is approximately ten times higher than the Florida state average for facility fines.
The facility received four 'Level G' citations, indicating inspection findings that resulted in actual harm to residents, including issues related to improper hospital transfer and discharge protocols.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

"I had my grandparent there, he loved the place. The staff are friendly, the other residents are friendly and I can only say good things"
"And please eagle lake do not text me back, I know you have nothing else to do except argue with people that give you bad ratings for no point wasting your time texting me when you should be taking care of my daughter and all the other patients."
"Amazing in every way as my daughter is new here and scared. The staff has been very supportive and welcoming. So far so good & just wanted to update anyone with our current experience. God bless everyone and please understand these nurse's need great support too as they care for our family members & deserve to be appreciated 🙏 Much love and Respect from this Family"
"Susan the Admissions coordinator was very helpful in answering my questions. I am looking forward to touring the facility"
"My mother has been receiving care at this facility for over a month, and I feel compelled to share my experience. The initial admission process was positive, with kind treatment from Denise and helpful assistance from the admissions team. Unfortunately, the overall quality of care has been disappointing. The agency staff at the facility have demonstrated a concerning lack of professionalism. There have been instances of rudeness and disrespect towards patients, including my mother, who has been subjected to unprofessional behavior and name-calling. Additionally, there have been occasions where CNAs have refused to assist her. Given the severity of these issues, I have decided to wait until closer to her discharge date to share this review, as I was concerned about potential retaliation. I plan to report these concerns to the state and the Ombudsman. It is distressing to consider the potential impact on other residents, especially those who may be less able to advocate for themselves."
"My mother is a resident at Eagle lake nursing home I live out of state. I went to visit my mother unannounced the place was clean and my mother is well taken care of. She loves it there. My mother is very happy there."
"Dead cockroaches on floor. Staff terrible not clean . Unsafe for patient care.daughter brought there. Nurses don't wear gloves when redressing picc lines. Pt laying for two to three days with blood on sheets from arm wounds. Terrible example of patient care. As a retired Respiratory Therapist for over 40 plus years. If I could give o stars I would. Trying to get my daughter out of there right now. She doesn't deserve treatment like that., nobody does. DONT TAKE YOUR LOVE ONES TO THAT PLACE,!!!"
"Such an incredible facility. The customer service is unmatched the staff deeply cares about their residents and makes every attempt to personalize care. The staff always made sure that my brother had his favorite magazines and deeply cared about his interests and our family wishes. This level of care is rare to see. I highly recommend this facility to everyone."
"I got a phone call from my grandma last night who is a resident at this facility. She was scared because the person in the room next to her was having a medical emergency and the use that was apparently calling 911 kept screaming at the operator about having to answer their questions about the person that needed a ambulance. She was very nasty towards the 911 person just because they were doing their job and trying to gather information for their ambulance people and even hung up several times from what my grandma said. I will be talking to my family about moving her to a facility that actually has a staff that cares about their residents!!! My grandma told me this nurse's name is LaShanda or something like that. She does NOT need to be in healthcare with a poor attitude like that."
"My father needed to be looked after and Eagle Lake has been the perfect home. He likes the staff and when I go in there everyone is welcoming and has a smile on their face. They will accommodate your needs and make sure your loved one is taken care of."
