    Summary

Maplewood Health Care Center is a 160-bed for-profit facility in Jackson, Tennessee, currently designated with a federal abuse icon and characterized by high staff turnover and a one-star health inspection rating.

    Detailed Analysis

Data from the Centers for Medicare and Medicaid Services (CMS) indicates that Maplewood Health Care Center holds a one-star overall rating, influenced by a health inspection score that is lower than the Tennessee state average. Records show a significant recent regulatory event: a March 2025 finding of'immediate jeopardy' related to the failure to protect residents from abuse and neglect, which resulted in a fine of $144,120. In the most recent inspection cycle, the facility was cited for 15 health-related findings and 12 fire safety findings, including issues with fire alarm maintenance and obstructed exit corridors.

Staffing metrics reveal that while total nursing care hours (4.17 hours per resident per day) exceed the state average of 3.85 hours, there is a notable decrease in staffing levels during weekends. Specifically, nursing care drops to 3.74 hours per resident per day on weekends, reflecting a reduction in available staff during those times. Registered nurse staffing of 0.58 hours per resident per day is slightly below the state average of 0.61 hours. Continuity of care is impacted by a nursing turnover rate of 65.7% and a registered nurse turnover rate of 54.5%, both of which are markedly higher than state benchmarks. Additionally, 13.7% of total nursing care and 14% of certified nursing assistant care are provided by outside contract agencies rather than permanent staff.

    Key Findings

The facility received a federal abuse icon following a March 2025 finding involving a failure to protect residents from abuse and neglect.

Health inspection findings increased from 10 to 15 in the most recent cycle, significantly exceeding the Tennessee state average of 5.8 findings per cycle.

Total nursing staff turnover is 65.7%, which is higher than the state average of 48.7%, indicating that approximately two-thirds of the nursing workforce left within a one-year period.

    Performance Trends (24 months)

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"When my mother broke her leg, she was transported to Maplewood after 23 days in the hospital to receive PT and OT. The entry and admitting staff were very pleasant, helpful and thorough. Therapy has gone well so far (after a week) and the nurses and care staff are always super friendly and are taking good care of my mom, getting her dressed every day and sitting up in the chair, as she was staying in bed most of the day at home. Would definitely recommend this facility."

"I was impressed as I walked thru the building to visit a friend. Numerous people spoke and smiled at me and my granddaughter. My friend told us that she was getting great care and liked it there if that’s what she needed to do now. No bad odors, except for the perfume or cleaning stuff that they used. I guess it could have been deodorizer. Sorry but I am allergic to strong smells. And I understand that sometimes it’s necessary.."

"Would not recommend to anyone. My mom spent one week here. One week too long. Food is terrible. The care is horrible. Rehab is a joke. The people living here looked unkept."

    Naomi Bennett★★★★2/16/2026

"Everyone was nice and courteous. Clean but could use a little spruceing up. Confident Mom is being well cared for."

    Terri Tyler★★★★★9/21/2025

"Safe. Helpful. Loving. Go out of their way to help you. Clean. I saw Dr Koontz and it made me so happy! He was my doctor once! My dad is safe there and was so welcomed! It made him happy. Thank you, Maplewood! We love you!"

    Eunice Glover12/11/2025

"The place is horrible. The staff is rude and disrespectful. The staff does not care about their patients. The staff talk about the patients and other staff members. That place is horrible. I wouldn’t let me dog stay there. I would hate to see how they treat their own parents. Overall a garbage dump. You know what is messed up? You can not reach the main corporate office. They don’t want to deal with issues cause it’s all about that money. They are just as bad as the staff that work at Maplewood Nursing Home. Corporate won’t even give a number so someone can reach out to them and let them know what kind of nursing home they are running. That’s sorry"

"Everyone was wonderful and helpful. The entire facility was spotless. I am thankful my best friend is in their care."
