    "My grandmother has been in here since the 19th of February. At first everything was great....until her bed kept messing up. For 4 days now it has not been fixed when the maintenance man said he fixed it. My grandmother's feet are terribly swollen. Everytime I come to see her I feel that she has been neglected. And when she pushes her call button I've noticed 1 time that it took at least an hour for someone to come see what was wrong. She's only here temporarily for physical therapy, maybe they do better with the permanent residents I hope. She won't come back here."

    "Our family had a wonderful experience with Silas Creek Rehabilitation Center after my family member’s accident. From the moment we arrived, the staff was welcoming, compassionate, and professional. The nurses and therapists went above and beyond to make sure he was comfortable and motivated every step of the way. The therapy team truly cared about his recovery and celebrated every bit of progress with us. The facility was always clean, cheerful, and had a real sense of community. It gave us peace of mind knowing he was in such good hands. Thank you, Silas Creek team, for helping our family through a tough time with so much kindness and encouragement. We’re so grateful for the excellent care and support you provided — you made a world of difference!"

    "It was terrible the 4 weeks I was in. First shift was decent. Second shift staff would be laughing, cackling, and cutting up in the hallway when my nurse light has been on for over an hour to go pee. My wound care was not regular, nor did I receive the antibiotics or pain medication I needed on time. Third shift was the worst. Still laughing and cackling instead of working. The cna on third shift left me in my pee and clocked out. I called the front desk to report her, and after that she was always short with me. Laundry was not being done consistently. Attitudes were the only thing readily handed out. I'm glad for the few people who had kindness but this place is from hell."

    "Our aunt arrived on 2/2/2026. My husband stopped by yesterday, and our aunt was still in the bed, no one got her up. Today, 2/5/2026, visited aunt. My husband asked for certain things Monday, 2/2/2026, and yesterday 2/4/2026 and noticed today they were not done. Also, today aunt was not dressed when he arrive at 11am.We did not visit on 2/3/2026, we know they didn't get her up and dressed. Same clothes still in her closet. When I got there today after my husbands visit, 2pm of course she was up and dressed. Everything he asked for was done by the time I got there. We should not have to do this! So people, pop in on your family at different times, or have a friend stop by for you. They gave my aunt a bath and her skin was so dry. They didn't bother to put lotion on her. Her skin was ashy. She is in your care!! I will discuss PT the next visit."

    "My husband was put into this facility and it was rated a 4 star. We thought with a 4 star rating, he should be good. In the beginning, everything seemed ok, but that went downhill fast. The daytime shift did pretty good, but gradually tampered off when it came to giving him his meds on time, bathing him, turning him and checking on him. Nighttime shift was even worse. Most of the time one of us visited him, which was a 5-6 times a week, we had to bathe him. I turned him and noticed he had a huge red spot above his bottom. I knew that wasn't right so I called a nurse in and she tried to make it seem like it was nothing and then proceeded to make everyone leave the room so she could take care of it. I stayed in the room and she asked me with a hateful tone who I was and I told her I was his wife and I was not leaving. I said why hasn't he been turned consistently. That's obviously a bedsore. She proceeded to say oh that's just an abrasion. She put some kind of cream on it and a patch. I told her again I felt like it was a bedsore but she just walked away. For about a week after that, every time one of us visited him, we would check and the sore was getting worse. They still wasn't keeping him turned or cleaned. His room stunk worse each time we came. My husband was fighting cancer, back surgery and trying to fend for himself, but could only do so much when he couldn't even walk. In the end, we had him sent home and the bedsore had to be cut on to get the dead parts off and in return the bedsore hendered my husband worse than anything. He died Feb.25, 2025 and the thing that was worse for him to handle was the bedsore that started at this facility because they did not take care of him like they were suppose to."

    "I tend to agree with Tina above. I was released to Silas Creek Rehab Center after my ileostomy because of their 5 Star rating and when I first arrived all was good until it wasn’t. I arrived on a Sat afternoon and received dinner that evening and breakfast and lunch the next day, but I was skipped at dinner time. When I inquired about my dinner tray the nurse told me that yes it was on them that I didn’t get a tray but I should have called during the dinner hour and let them know I had not received my dinner. I had a guest during that time and had no idea that it was past the dinner hour until my friend had left and I found out the time. Ultimately it was not my responsibility to keep up with that kind of thing. When they did bring me a snack it was mostly things I could not eat due to my restrictive diet. During my stay I was never, not even once, given the diet that was required for me and even after a meeting with the director of food services and giving him a copy of my diet restraints and needs did I receive what I needed. The Ist shift care was good, but as Tina said, 2nd shift went down and 3rd shift was the worst. I guess the final blow was when my bag had sprung a leak and I needed help in changing it that the nurse came in laid on the bed all the materials needed to change it then said, “we’re supposed to be teaching you to do this so here do it. Then she pulled the curtain partially and left the room. While I was trying to do this my roommate’s father came to visit and during his visit he pulled back the curtain without a reason and started to say something to me and there I sat, stark naked for all to see. I’m afraid that the facility’s report of my stay and mine were vastly different. I’ve been in rehab before several times and this was not a good experience."

    "The nursing staff and CNA’s were kind and attentive. Housekeeping was attentive and kind to my mother. Mom said the food was incredible! The PT, OT and physician were prompt, kind and knowledgeable .The only downside was social work saying one thing and doing another and also those working on discharge said five minutes all day which turned into hours and hours. Also what was told at discharge about follow up was not the same when I called the social worker back the next day. Overall, the staff seems to care about the patients as well as the management seems to care about the staff."

    "When my mom first came to this facility they weren't perfect but now they are awful. And the administrator is the worst. They will lie to you straight in your face about your loved one refusing to be turned every two hours or bathe. She is not refusing they don't have enough clean linen. I had to go purchase my mom her own towels and washcloths because the workers would tell her they don't have anything clean. The administrator told me they saw on camera dirty linen in the hall in the middle of the night indicating she had been turned and changed. I asked to see the footage but of course she started quoting Hippa laws. What Hippa law are you breaking if there are no residents in the hallway in the middle of the night? Miraculously, after I said call me every time she refused (no matter the time) she started getting turned and changed. She (Kendra) never return calls and makes up all types of excuses of why she didn't return your call. I even asked her to come see me in my mom's room when I was at the facility. I waited 3 hours and she never came. She has seen me at least 2-3 times since then and has not once approached me. I also sent her an email with no response. I have decided not to call her anymore, I will just communicate with "2 Wants to Know" the next time I have a complaint and I will have pictures to back up the filthiness of the facility and all the other issues. I clean my mom's room MYSELF every time I come there. My mom is now on hospice care in the facility and I cry when I go, I cry when I am there and I cry a river when I leave because I don't know what kind of care she is going to get when I leave her there. I am starting to feel depressed just having to leave my mom in a place where administration doesn't give a damn. Every day I am emotionally exhausted because I work and then have to go to Silas Creek and be a guard dog or send somebody for me. Most of the workers seem to be trying but they are overworked, having to take care of 15-20 patience at a time. It is sad how these patience have worked all their lives and have to turn over all their assets but $70 to get treated with disrespect, cruelty and a lack of dignity."
