Diversicare of Big Springs demonstrates improving inspection results and a stable, permanent workforce with zero agency use, though total nursing hours per resident remain below state averages.
Analysis of Centers for Medicare and Medicaid Services data indicates a period of operational stabilization at Diversicare of Big Springs. Specifically, care from registered nurses is recorded at 0.58 hours per resident day, compared to the state average of 0.64 hours. Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"I am writing to formally express my profound disappointment and serious concern regarding the care my 80-year-old mother received during her stay at Diversicare. Despite her being at the facility for over a week and showing steady daily improvement, it became painfully clear that the staff was unaware of her critical medical condition and history. My mother, admitted due to fluid accumulation around her lungs caused by heart failure, was treated as if the issue was unknown to the nursing staff. Although she rarely complained—except about the food—they failed to investigate her pain appropriately. When I visited with my brother, it was obvious that the nurses did not understand why she was there. I had to explain her condition and medical background, and only after insisting was an ambulance called within ten minutes. Upon hospital admission, 40 to 50 ounces of fluid were removed from her lungs—an alarming fact the facility had completely overlooked. From my personal experience caring for family members with serious health issues, I believe the operations at Diversicare reflect a disturbing disregard for patient welfare. The staff seems more focused on deflecting responsibility than addressing genuine medical needs, especially for vulnerable patients who "never complain." While some nursing staff appear to care, they seem silenced by those with more experience but no empathy. The lack of proper care and attention I witnessed is unacceptable and, frankly, comparable to some of the worst neglect imaginable. I strongly urge that Diversicare’s practices be thoroughly reviewed and that serious reforms be implemented. This facility currently prioritizes convenience over quality of care, putting vulnerable individuals at significant risk. The neglect and ignorance I experienced lead me to believe that, had my brother and I not intervened, my mother's condition could have had fatal consequences. It is imperative that this facility no longer be allowed to operate without significant changes to its approach and oversight. The exploitation of patients who are not wealthy is utterly unacceptable, and I am committed to advocating for the closure of such negligent institutions. Thank you for your attention to this critical matter. I look forward to your response and a detailed account of the steps that will be taken moving forward."
"This facility was one of the worse places I have EVER had to visit. My husband was there a few months ago and he kept getting infections because the staff wasn't cleaning and changing his bandages properly. My husband fell there twice and he had to tell me he fell. The staff said nothing until I asked them about it. I tried speaking with management about the situations that kept occuring and all I was gave was excuses. Half of the staff there is rude and very uncaring about the people that live there or receive any treatment there. I wouldn't leave my worse enemy there. I even overheard nurses laughing and making jokes about certain patients there that hadn't received baths in months. If you have family members there Please check on them regularly because that place is horrible."
"My mother who has dementia had a fall at home and she requires rehab. I am a Licensed Nursing Home Administrator in Rochester NY. My father asked my to help choose her rehab. We settled on Big Springs because they are a 4-star facility, staffed well and have an amazing rehab department. All nursing staff have been amazing and tentative to my mother‘s needs. Based on our experiences I would suggest Big Springs. My mom is safe, and receiving the care she needs to return home. Thank you to all the staff at Big Springs!"
"The people who work there are very caring. They are kind and want them to be comfortable and taken care of. Lexy rocks and there have been multiple others that have been so good to my mother. Eva, lucinda, bronma ????? I wish I knew all the names!!!! I would totally give a shout out to all the ladies and the 1or 2 guys if I knew their names!!!"
"If I could give this place a zero star I would. The management is Very Unprofessional. They don't care about pts nor staff. They Stay understaffed and aides overworked. The facility is nasty and disgusting they don't like to use any disinfectant. They leave the residents pissy for hours and when you ask the nurse they claimed someone had changed them. I wouldn't leave my enemy in this place. They have multiple lawsuits, mold in the walls, maintenance workers who don't know what they're doing. This place needs to be shut down with the health department and state going through there."
"Similar to others here, my family member contracted MRSA in his first 72 hours in the facility and had to be taken back to HH ICU. He was discharged back to Big Springs a week later. On his second day back, I went in to see him early one morning. His bed was all the on floor and he was on the bed in a soiled diaper with no other clothes or bedclothes and he was cold, lying sideways across the bed. We got the nurse in there to show her and she remarked that she left him like that because he tried to get out of his bed. After just a few days back at this facility, he again had MRSA so badly that it because a case of septic encephalopathy. At this point we do not even know if he will survive. This place should be shut down immediately. Worth mentioning that on our first visit there, we were briefed about his condition, only to find out days later they were reading from the wrong patient't chart. DO NOT SEND YOUR FAMILY TO THIS PLACE! - My relative was here to rehab from a surgery. Now he may never go home. Think about it before you let a Hospital Social Worker tell you this place is good."
"Absolutely horrible place. They left my dad in a soiled diaper with an open bed sore. He just got over a MRSA infection, and he is a diabetic. He would call for help and they either wouldn’t come at all or it would be 45 mins to an hour later. They did not give him his insulin and his sugar got extremely high, and they still didn’t give it to him until much later. He was without his pain medication for 18 hours. They were extremely rude to my mother, who was nothing but nice to everyone. We overheard one of the employees yelling at one of the patients. There is a difference in being stern and being hateful. These people are definitely hateful. If you can’t do your job right, and have empathy for those that are elderly and sick you should not be in the healthcare field. This place should be shut down. My dad can speak and if they are treating people that can speak for themselves this bad, I can’t imagine how they treat the people who are unable to speak for themselves or take care of themselves, and it makes me sick. Do not take your loved ones here. It’s a disgrace."
"My mother was admitted to Big Spring Rehab for a short stay to regain strength after a short stay in the hospital for a UTI. Wednesday June 19th was her 1st full day, by Saturday, 3 days later, she was 10 times worse with confusion and mobility. During her stay, she was not given her phenobarbital which she has taken for 30 years due to brain tumor surgery and was not given her pain medication which is a daily medication she has taken for years due to osteoporosis. both should not be just stopped cold turkey. when I asked about it, all I got was excuses and nothing resolved. I witness my mom laying in a urine soaked bed and feces!! With a pullup full of poop laying next to the bed in the floor. I walked up and down the hall looking for someone that worked there and there was no one It was absolutely horrible and a disgrace for someone to be treated that way. no one would ever come when she pushed button she screamed for help no one came. This was on saturday, Sunday morning, I received a call at 6am that my mom refused care, I am like What CARE!! I went and had to call an ambulance to take my mom to Hospital. she was having black outs and terrified of the head nurse!!! My mom was traumatized by what happened to her. she went just for rehab and ended in another hospital stay. She has not regained her ability to walk with her walker or her mental status, as she was prior to going there. My mother did not have dementia!!!!!! There is so much more, that I will leave for our attorneys. They are all liars and neglect their patients. Please, Please do not put your family member here!!!! I feel so sorry for the ones that cannot speak for themselves! I will never understand how this place can continue to stay open!!!!! everything about it is unprofessional!!!"
"I wish I could leave a review and give them no stars. DO NOT put your loved ones in this place. They are extremely understaffed and outdated. Patients have to sit in their own urine and accidents. The call button will go unanswered for over 30 minutes. There’s not enough staff for the patient load of the facility, and that’s dangerous. If your family does end up here, god forbid you ask about filing a complaint. My family did that and was later told my father would be discharged due to insurance coverage. Remember, a social worker arranges everything and CONFIRMS insurance coverage before you ever set foot in the building. As you can probably guess, a social worker later told us everything was fine with our insurance and my father was covered. The moral to the story of our experience? PLEASE find a better facility for your family members."
"I wish I could rate less than a one star. My mom was there for 10 days. She only received 1 bath during the stay, even after requested one the whole time. The never changed out her pick line for her antibiotics. Should have been changed twice. The first 3 days she didn’t receive any of her medications. Including her blood thinners that she is on for life due to open heart surgery. 9/10 nights they forgot to bring her dinner and she would have to call for hours to get someone to bring her something. I came up there, I was buzzed in by a random guy that was walking by. I had my moms stuff in a backpack. No one asked who I was seeing or where I was going. Walked thru, never stopped. Only 2, I mean 2 nurses for the entire building. When discharged with no notice at all. I got there and no one seemed to know what was going on and said I would have to come back later. They ended up getting her out within 10 minutes.I didn’t have to sign anything, no discharge paperwork. No set up for home health services. Not to mention when I was trying to make sure I didn’t need to be buzzed out, if course no staff around, the entire side smelled of urine. They have been contacted by the home health now taking care of her, they have none of her medication or care at j their system. Please don’t put anyone here. It’s horrible."
