The data profile for this facility is characterized by a one-star federal rating, an active abuse alert, and a health inspection finding frequency that is significantly higher than the state average.
The data indicates that Pacific Villa, Inc. operates with a one-star overall rating, the lowest possible designation from federal regulators. This score is primarily driven by a high volume of health inspection findings, which increased from 27 to 33 in the most recent cycle. These findings include failures to implement measurement-based care plans and failures to immediately notify physicians and families of resident injuries or clinical declines. Furthermore, the facility has an active abuse alert icon, signifying public records of incidents involving potential harm or neglect. Financial penalties have also escalated, with nearly $54,000 in recent fines and a mandatory denial of payment for new admissions lasting nearly a month.
Regarding staffing, the records show 3.96 total hours of nursing care per resident per day, which is lower than the California state average of 4.51 hours. Registered nurse staffing levels are notably low at 0.22 hours per resident per day, compared to the state average of 0.65. While total staffing hours have increased by approximately 12% over the last two years, the quality of care provided on weekends shows a decrease, dropping from nearly 4.0 hours during the week to 3.64 hours on Saturdays and Sundays. Nursing staff turnover is currently at 39.1%, which aligns with state averages but indicates that approximately four out of every ten nursing staff members leave each year. Continuity of care is supported by a low reliance on contract or agency staffing, which represents less than 1% of the total nursing hours.
CMS has issued an active abuse icon for this facility, following inspection findings related to the failure to protect residents from abuse or neglect.
The facility was cited for 33 health-related inspection findings in the most recent cycle, which is more than double the California state average of 15.6.
Federal records show four recent penalties, including a fine of $48,100 and a 29-day denial of payment from Medicare in April 2025.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"Pacific Villa Nursing Home stands out for its compassionate care and dedicated staff. The nurses and caregivers are attentive, patient, and genuinely caring, creating a safe and supportive environment for residents and families. The facility is clean, well-maintained, and communication is reassuring. I highly recommend Pacific Villa to anyone seeking quality care with dignity and kindness."
"I’m trying to get my sister transferred closer to me. I left two messages on social workers ext. finally got to talk to Nancy the Social worker. I said I’ve left two messages on her ext nd she said she doesn’t even have voice mail set up yet you’re led to believe there is that option. Apparently she’s the only social worker for 99 beds. Also left two messages for Trevor Anderson, administrator and never got a call back. These SNF’s are worse than the dog pound. Should be shut down."
"* They put a false statement in my sister's chart saying she struck at nurses, so she was rejected by another nursing home; * 2 staff interrupted me to argue with me; * the voice mail for most phone numbers picks up, but no one ever calls you back because it is disconnected. My sister complained about a medicine's weight gain effects after gaining 70 pounds in 1.5 years.and was ignored for months; *The Director of Nursing called me back one time in 3 years. But their food is not bad."
"If you love your family member, do not leave them at Pacific Villa. The nurses never leave their nurses station. The quality of care is extremely poor. My brother almost died when he was given medication that should not of been taken with his regular meds. But no one thought about checking the medication compatibility. Money and his personal belongings were constantly missing. And they will lie about anything you ask trying to keep themselves in a good light. Terrible place."
"Smoking in patio only. NO WI-FI FOR GUESTS. Soda and junk food vending machines available 24/7. Expect to use your data so know your limits. Food good but bring your own salt and toiletries. Eggs at breakfast are never salted and are given every day. Games and chair exercises nearly every day. Tell them if milk makes you gassy so they can provide alternatives. Sleep with earplugs to avoid noisy guests and bring your own books. Good luck."
"Friendly Staff. My nurses JoAnn, Mia and Erica have been terrific. They gave me the Someone's Been Naughty t-shirt from their Christmas party even though I was brand new and will only stay until January."
"Well they wouldn't let me see my mom Debbie over a cover 19 test and I am sad upset right before the holiday. They don't think about how people feel. It almost christmas and I am upset and I wish I could see her."
"This place is so unprofessional they treat patient with no respect and care they like to cause problems for the patient if anything don't go their way they will call the police and let them handle it what a unprofessional place they neglect the patient and don't care about their needs whatsoever the employees are very rude and don't respects the patients belongs it is a big mess I highly recommend you don't sent your loved one there they only care about money not people"
"I haven't had anything stolen. The nurses take good care of me."
"Clean facility. Vangie is an excellent DON!"
