Kalispell Rehabilitation and Nursing LLC is a Special Focus Facility with a one-star federal rating, characterized by lower-than-average staffing levels, substantial financial penalties, and a high volume of health inspection findings.
Kalispell Rehabilitation and Nursing LLC currently operates under the federal Special Focus Facility designation, a status reserved for nursing homes with a history of significant care issues. According to federal data, the facility holds a one-out-of-five star overall rating. The most recent health inspection cycle recorded 24 findings, which, while lower than the previous cycle's 47 findings, remains significantly higher than the Montana state average of 11.5 findings per cycle. Serious findings include failure to prevent accidents and failure to develop measured care plans for residents.
Staffing data reveals that residents receive an average of 3.06 hours of total nursing care per resident per day, which is nearly an hour less than the state average. Registered nurse care is 0.54 hours per resident per day, notably lower than the Montana average of 0.94. While the facility does not rely on temporary contract or agency staff, it experiences a high nursing staff turnover rate of 42.5%, with half of all registered nurses leaving within a year. Additionally, total nursing hours drop by approximately 10% during weekends compared to weekdays. The facility is part of a 18-member chain with an average overall rating of 1.6 stars, indicating performance trends consistent with its corporate affiliates.
The facility is designated as a Special Focus Facility, a federal status indicating a persistent pattern of poor performance that requires increased oversight and more frequent inspections.
Records indicate four recent financial penalties totaling $326,610, including a single fine of $245,440 in late 2025 following health inspection findings.
Direct nursing care hours of 3.06 per resident per day are approximately 23% lower than the Montana state average of 3.97 hours.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"This place is horrible! If you can at all try to find another place for your loved ones eles where do it.Filthy dirty disgusting building horrible and I mean horrible care! Try to take anything they might have and threatened them that they will throw them out if they don't sign over every thing they have. And even try to take my ex husband's coffee craft I purchase for him saying he stole it from the kitchen! They have done this 3 times. Bad enough they charge these people 20, 000.00 a month to stay in that crap hole. I'm going to try my damnest to get him out of there! I wouldent even feed my dog the crap they try to feed those people!!!! Out date spoiled crap and apple juice that is fermented! Disgusting 🫣 🤮"
"KRN as we call it is a very good care facility, the staff and admin is great and does a very good job. My mother was cared for very well last year post her hip replacement and for her cognitive decline/dementia/Alzheimers. I would go back there for my mother to be cared for at the right time."
"Just sent my Grandmother here recently! After reading some of the reviews on here I am a little worried....... Hopefully they take good care of my Grandmother! Only time will tell. The family & I will be visiting her quite often! I hope they keep my Grandmother clean, comfortable & fed 🙏"
"Id recommend sending your family literally anywhere else.. There are ants all over the facility, in residents rooms, it's disgusting. Residents can't have any nice clothes because they end up getting lost. It's completely unorganized. They do not care about quality, they truly only care about how much money they can make.. it's sad and I hope for the Residents sakes that this facility cleans itself up."
"WORST care ever. Husband allowed to fall with CNA (had not worked with him before) in the room hitting his forehead which resulted in a brain bleed one day prior to discharge. Understaffed, no MD available readily, terrible pericare, should be fined for ALL of the falls."
"My mom who had advanced dementia was in the dementia unit following a broken leg in December and January. The staff in that unit so kind and caring. My family also felt that they took excellent care of her."
"Not a place for my mother to be. Unfortunately she is there for a few more days. The staff will not inform me about my mother's health and issues that she is having. I tried a number of times to have the Dr or physical therapist call me, or come out of the office to talk to me about her health, and what we can do to make her stay as pleasant as possible, and what we can do to help her get back home... With no avail, absolutely no communication from the staff. I would recommend finding a more suitable place for your loved ones, They are very under staffed there. Could be the reason for that kind of treatment. It really makes me SAD."
"DO NOT let your love one be placed there!!! My husband was placed there after being in intensive care for a week. The bed was barely adequate, the TV didn’t work, there was a very unsafe broken chair in the room. He wasn’t fed regularly, He was promised baths that never happened. The nurse was overwhelmed. Medication was not given at the proper time. The lack of compassion and care was unbelievable!!! I took my husband out after a week… do not let the new name fool you, Kalispell rehab and Heritage Place are the same facility."
"My grandmother was placed in the his facility with COPD. We showed up to visit and found her laying in bed with the emergency cord wrapped around her hand so tight it was cutting off circulation. She was laying in soiled clothing. She had to have payed there for some time like that because her clothes were drying. We were outraged! We filed a complaint and asked for answers but received a list of excuses. Within a week we had her moved. A couple years later my grandfather was ill and transferred from the hospital to Heritage (only opening at the time) until an opening at Brendan house came open. In 6 weeks he was treated horrible. We are talking the most gentle man you’d ever meet. I visited often and after a couple weeks I noticed a shift in his personality. He quietly opened up about a mean nurse. He watched his door was and spoke very quickly while telling me. He lifted his shirt to reveal horrible bruising from insulin injections. He said she would not listen when he asked to have injections moved around (alternating injection spots) because of the pain he was in. He also showed me bruises on his arms from her. She told him to “quit whining and act like a grow. Man.” I was so afraid to leave him there. He was also terrified of me reporting them, fear of retaliation, if she found out he told on her. Our elderly should never have to endure this type of treatment. This was absolutely abuse/neglect and nothing was done about it. I did file a report and did everything in my power to get him moved ASAP. I was able to get him moved to the Brendan house where he was much happier. There is a reason there’s typically opening available at this facility. Keep that in mind when looking for somewhere for your loved one."
"I have been at Heritage Place for a year and a half. I can't thank them enough for the care they have given me. I spent 10 days in the ICU with a very serious illness and when I came back I couldn't move or walk , they gave me such extra and personal care. Thanks to physical therapists and the encouragement of CNA's. My bathes are always regular. As well as daily care by staff that is loving and supportive , responsive to my needs. I am grateful to be here. Another incentive to Heritage Place they are one of the few who take Medicaid. I don't what low income people would do without them. They have increased their pay to be competitive. The new Administrator is making great strides in making this a better place. We are all a family here."
Federal and state inspectors conduct regular surveys of every nursing home. Deficiencies listed here are problems found during those inspections.
