Records indicate a facility with strong short-term clinical outcomes and positive health inspection trends, contrasted by staffing levels and nursing hours that fall below state averages.
Analysis of federal data reveals a significant disparity between health inspection performance and nursing labor levels at this facility. While the facility maintains a high health inspection rating of four out of five stars and has reduced the number of standard inspection findings from six to three over the last two cycles, staffing metrics remain a point of statistical concern. The facility provides 3.07 total hours of nursing care per resident per day, which falls below the Georgia state average of 3.57 hours. Furthermore, registered nurse staffing levels—at 0.35 hours per resident day—are notably lower than both state and national benchmarks.
The data further highlights staffing volatility and historical penalties. Nursing staff turnover is elevated at 47.4%, and there is a 9.7% reduction in nursing care hours during weekends compared to weekdays. Recent inspection records from October 2024 include findings related to the protection of residents from abuse and neglect, as well as requirements for timely reporting of investigations to authorities. Additionally, fire safety records show twelve uncorrected findings, including issues with automatic closing doors and accessible exits. While the facility's short-stay quality measures are rated at the highest level (five stars), the long-stay quality measures receive the lowest available rating (one star), indicating a statistical divergence in outcomes depending on the duration of a resident's stay.
Reported 0.35 hours of registered nurse care per resident per day, which is approximately 28% lower than the Georgia state average of 0.49 hours.
Total nursing staff turnover reached 47.4%, which exceeds the state average of 46.5%.
The facility incurred a $41,316 financial penalty and a five-day payment denial following a February 2024 inspection.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"This place has been great for my brother. He has been here since March and so far Everyone has been great. When I call to check on him they always answer all my questions. My mom and I go see him weekly he is clean, shaved, hair cut. He has came along way from when he 1st got here"
"This place is horrible! A dog pound smells better. They do not clean the patients. A loved one was sent here due to hospital mix up on rehab homes. I had to demand them release my loved one , they refused at first. ( we paid for the ambulance ride) Patients were sleeping on the floor on make shift cots. It was something you would see in a third world country! Patients walked around with soiled clothing and Pooping in the hall! I had never seen anything like it in my life. Do not send your loved ones here.It doesn't deserve 1 star it needs to be shut down!!!!!!!"
"Who ever wrote the good review must work for them, This place doesn't do much if anything for the people depending on them, How any human can treat another in such a way is hard to understand. Walking in you gag from the smell the food is not something the dogs outside will eat. It makes me sick knowing people are being treated this way."
"If you love your loved one do not place them here!! My father in law went from smiling, happy and full of life to being in his death bed in 7 months. I suspect negligence but since we live so far away we can't prove it. Horrible staff and very unprofessional! Please for the love of your loved one look somewhere else!!!!"
"I have had a wonderful experience. The staff is kind, caring and compassionate. The facility is very clean. The negative reviews I saw definitely do not represent my experience."
"this place is horrible, my elderly neighbor was placed there against her will. and the workers are saying that she is not allowed to sign her self out she never gets to see any daylight or anything"
"This place doesn't deserve any stars, absolutely the worst most disgusting place I've ever seen! This was in a resident's room.(it's their dirty cloths with maggots crawling out)"
"THIS IS THE MOST UNPROFESSIONAL ESTABLISHMENT I HAVE EVER ENCOUNTERED. THE STAFF ANSWER THE TELEPHONE LAUGHING AND SOUND LIKE THEY ARE EATING AND HAVING A PARTY. WHEN I TRIED TO SPEAK TO UPPER MANAGEMENT, I AM GIVEN SOME PERSON THAT IS RUDE AND COMBATIVE AS THOUGH SHE IS ANGRY ABOUT SOMETHING AND VERY PROVOCATIVE. THIS PLACE REALLY NEEDS TO BE INVESTIGATED. I HAVE TIRED TO COMMUNICATE WITH THESE PEOPLE WITH NO AVAIL. PLEASE BE CAREFUL IF YOU HAVE LOVED ONES UNDER THEIR CARE."
"This is place should be shut down, the staff is very unprofessional and the facility is unclean there is no reason why the residents should be in bed at 3pm and not doing some kind of activity"
"Very poor communication with staff and and between department"
