Aventura at the Bay is a federal Special Focus Facility characterized by high inspection findings, significant financial penalties, and nursing turnover rates that exceed state averages.
Public records indicate that Aventura at the Bay is currently designated as a Special Focus Facility, a status maintained for providers with chronic patterns of poor performance. The data shows significant challenges in staffing stability and consistency. Total nursing staff turnover is 64.3%, and registered nurse turnover is 67.7%, both of which are substantially higher than the Florida state averages of 42.3% and 45.5% respectively. Furthermore, there is a marked disparity in care between weekdays and weekends; registered nurse care drops from 0.55 hours per resident per day on weekdays to 0.20 hours on weekends, representing a 63% decrease in available professional nursing oversight during those times.
Financial and safety records further reflect these challenges. The facility has been assessed $408,704 in federal fines over the last three years, which is significantly higher than the state average fine of $30,917. The most recent health inspection in August 2025 identified 23 separate findings, including failures to inform families of resident declines and failures to provide safe pain management. Additionally, the facility reported two administrator departures in a single year, which is a common indicator of management instability. Use of contract or 'agency' staffing is also elevated at 19.6% of all nursing hours, which suggests a reliance on temporary staff rather than permanent employees. Overall, the data illustrates a pattern of high inspection findings, staffing volatility, and significant regulatory penalties relative to both state and national benchmarks.
The facility is designated as a Special Focus Facility by federal regulators, a status reserved for nursing homes with a history of persistent safety and care quality issues.
Health inspection records reveal 23 findings in the most recent cycle, including failures to follow care plans and provide appropriate pain management, compared to a Florida state average of 7.3 findings.
Nursing staff turnover is recorded at 64.3%, which is 22% higher than the state average, alongside a 63% reduction in registered nurse hours during weekend shifts.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"I enjoyed several very good experiences during my residency at Aventura at the Bay. Some, even life changing. In my business and my personal lives, I believe all change is a result of an "impact". For example, it is a sunny day and then some impact changes all that in a millisecond. Health was the start of my adventure at Aventura at the Bay. My impact, resulting in my becoming a resident, was a condition that required my right leg be amputated. The full book is in work and will be available at our new web site, The "Cool Kids of Aventura at the Bay" at thecoolkidsofaventurabay The staff were friendly, patient, and genuinely caring with the residents. Any time I had a question, someone made the effort to explain things clearly and make sure I felt comfortable with what was happening. The building is clean, the atmosphere is calm, and the team works hard to keep residents engaged and supported. I appreciated the way they treated people with respect and kindness. Overall, I’m grateful for the care they provided and would recommend this facility to others looking for a supportive place for their family member. While I do not have ALL the names I wish to give a special THANK YOU to. More will be posted on the site. ***** Debbie Lord I was in another facility when I found Debbie. Debbie and her assistant Rose visited me and assessed my needs. To include accommodating my power chair. THANK YOU, Debbie. ***** CNA Desi, D WING Desi was my first CNA. Not nearly enough space to offer even half of the accolade she deserves. I love you Desi!!! And THANK YOU. ***** RN Erica Erica is yet another example of not enough space to offer my compliments. Thank You Erica! ***** Front Desk Vance I had ALOT of Amazon deliveries. Vance always saw to it that they were delivered to my room. I consider Vance one of many friends made during my residency. THANK YOU, Vance. ***** Dining Room, PAM Oh my! Pam is my coffee savior. I would seek Pam out at 5:30 AM if the wing pantry was out. I must have my coffee and Pam made sure that happened. THANK YOU, Pam. *****Business Office Kim "Ms. Kim" is another outstanding example of the TEAM at Aventura. Kim assisted me in any business-related matters. She would offer direction by virtue of her caring and vast knowledge of her profession. "Ms. Kim" is my angel. THANK YOU, KIM. While I know many that need to be added to the above list, I will post a more detailed history of my residency at Aventure at the Bay on the site. Remember...... Always say "THANK YOU" James Aitken CEO, SYSTEMS~ALPHA"
"My aunt became a resident of Aventura today. I had to sign in as a guest. I was very impressed with Advanced Entry. The system made me feel that my aunt was safe within the facility. The took my picture, name and who I was going to see. It reminded me of going through customs at the airport . I was told that once the initial information was entered, I would just have to show my face in the camera for entry at me next visit. It saves time and a sense of security."
"This facility is being run in a way that is not just disappointing , it is dangerous. My mother has been there for approximately a month. She has progressive MS and was discharged from the hospital with documented swallowing difficulties and severe limitations in hand strength and coordination. She cannot lift her hands properly or grip utensils, and she requires assistance with all activities of daily living. This is clearly documented in her medical record. Despite this, meals are delivered cold and she is given no assistance to eat. For someone with dysphagia, that is not just neglect it is a choking risk. The facility relies heavily on diapers and enforces what appears to be a rigid “two-hour rule” for changes. No human being should be forced to sit in feces or urine because a clock says it isn’t time yet. When my mother presses her call light, she has waited 45 minutes or longer while soiled. That is inhumane. We were also informed that the call light system only triggers a visual light above the door there are no audible alerts or direct staff notifications. In a facility housing medically fragile patients, that is unacceptable. If someone begins choking, falls, or experiences a medical emergency, it could easily go unnoticed. Sitting in feces and a wet diaper is not simply uncomfortable it is a serious health hazard. For an immunocompromised patient at high risk for UTIs, it can lead to sepsis. My mother reported UTI symptoms 3–4 days ago, and no one has catheterized her to obtain a urine sample for testing. Delaying evaluation and treatment under these circumstances is negligent. This is not solely a criticism of individual nurses or CNAs. No staff member should be forced to run room to room in a chronically understaffed facility where patients’ basic needs cannot be met in a timely manner. Chronic understaffing in a skilled nursing facility is a systemic failure. It compromises dignity, safety, and health. Beyond these major issues, there are also concerns about staff attitudes, cleanliness, and lack of adequate fall-risk accommodations. But those concerns pale in comparison to the pressure sores my mother developed after being consistently left waiting for care. This facility does not appear to operate under safe staffing standards. It creates an environment that is unsafe for patients and overwhelming for staff. If you have other options for your loved one, seriously consider them. What is happening here is not just poor service it is a safety risk."
"I have had the pleasure with the wonderful place. As i walk into the secure entrance, I am greeted with smiles from the front desk lady with deep blue eyes and a beautiful person. From the nursing staff, dietary and the management. Kim in the business office is AMAZING. She is always so happy and it shows that she truly loves her job and the people that live there. She is amazing. Also the lady who does tours, is pretty good herself. I would live here if i could. Please go check the place out. These reviews are not real"
"I visited this facility to visit an old friend of mine who has been a lifelong local missionary. This place has a very loving feeling to it. They are very careful about who they admit, but very courteous. They’re taking good care of my friend. The building was clean and everyone there seemed happy though they were often in wheelchairs ."
"Aventura at the Bay is a great facility for my father requiring short term rehabilitation. He actually needed back to back short term stays due to a recurring medical concern. The staff there was very kind, compassionate, and patient. My father was very well taken care of on both occasions. Thank you Aventura for the healthcare my father received by your staff at your facility."
"This was actually my 2nd visit. My family member was being transferred at the first visit so I didn't get the chance to visit. This visit was really nice. A great check in system, clean facility and pleasant smell. It's was during a thunderstorm and I stayed for hours which my loved one was taken care during the time. Her needs we're meet and I was pleased."
"My father is currently at Aventura receiving physical therapy. Everyone from the front desk to the nurses and the CNAs have shown nothing but professionalism, courtesy and compassion. My heart can rest knowing he's in good hands. Now that we are two months in I had to delete two stars. A blodclot went unreported for about 3 days before someone called the hospital. My dad's leg was triple the size and we had to advocate for him before anyone took action to get him hospitalized. I guess they were putting their best foot forward initially when I gave 5 stars. I or a family member goes every evening after work to make sure he eats dinner. His tray often gets dropped off and no one assists him or even helps sit him up in bed so he can eat. He also has a bed sore since becoming a resident there. I'll post another update soon. Hopefully, they don't lose all their stars and my dad gets improved care as he recovers from back surgery."
"This place is so nice. It’s clean and the staff is amazing. I love how automated the entrance is. You just stand in front of the camera, answer a few questions, they print out your photo and you’re done. On your way out, stand back in front of the camera, it scans your face and you’re done, signed out. It’s a beautiful place and I’m so glad my uncle is happy there and he said the food is so good."
"Dear Aventura At The Bay Staff, I am writing to express my sincere gratitude for the excellent care my husband received while he was a resident at your facility. The dedication of the therapy team was remarkable. They provided outstanding support, and I truly believe their efforts were instrumental in his recovery. I would like to extend a special thanks to June, Tyler, and Evan for their commitment to helping him regain his strength. The nursing staff on C-Wing was also exceptional. They provided compassionate care and treated him like a member of their own family. Furthermore, we were very pleased with the cleanliness and maintenance of the facility. While there is room for improvement with the food service, the quality of care was truly outstanding. Thank you again for your hard work and kindness."
