Records indicate a facility with nursing turnover and contract staffing rates exceeding state averages, alongside registered nurse staffing levels that are lower than the Vermont average.
The data indicates a facility with fluctuating performance across several key operational metrics. CMS records show that registered nurse staffing levels provide 0.36 hours of care per resident per day, which is significantly lower than the state average of 0.81 hours. Furthermore, the facility demonstrates a high reliance on temporary agency labor, with over half (52.9%) of all nursing hours and nearly 60% of certified nursing assistant hours provided by contract staff. This indicator, combined with a 69.2% nursing staff turnover rate, suggests frequent changes in the individuals providing direct care.
Inspection records reflect 10 findings in the most recent health survey, a decrease from 13 in the previous cycle, though 11 findings remain linked to formal resident complaints. Findings include issues regarding food safety, infection control protocols, and the availability of working call systems in resident bathrooms. Financial records show three federal penalties over the last three years totaling $47,258, including a 46-day denial of payment for new admissions. While total nursing hours per resident day have increased by approximately 13% over the past two years, the consistency of care remains impacted by staffing transitions and a high volume of uncorrected fire safety findings.
Records show a high reliance on contract personnel, with 52.9% of nursing care and 58.5% of certified nursing assistant care provided by temporary agency staff.
Registered nurse staffing levels of 0.36 hours per resident per day are less than half of the Vermont state average of 0.81 hours.
The facility reported a total nursing staff turnover rate of 69.2%, which is higher than the state average of 59.6%.
This shows how the facility's federal ratings have changed over the past 24 months. Consistent high ratings suggest stable quality, while declining trends may warrant further investigation.

Staffing depth is the strongest predictor of care quality. Facilities routinely cut staff on evenings and weekends — the shifts when falls happen and call lights go unanswered.

"I am very disappointed with the services at crescent manor. My family member was brought there for short term rehabilitation after surgery, they expected him to be there for 2-3 weeks. Since being there: no one checks on him for hours at a time, they lost his D/C paperwork and missed a follow up appointment with his surgeon, they do not try to get him up and moving at all, they can’t seem to get his medications figured out and he is missing several, his diabetes was controlled and now they are the highest they have been in 2 years. At this point he’s been there for 2 weeks and has declined the point where he is a Hoyer transfer and I don’t know if he will able to come home."
"My mom was a resident at CMR - she got great care and all of her needs were met. She loved the activities! She loved the food! The staff was attentive and compassionate !!!!"
"Administrative staff found it acceptable to yell at an employee in front of residents and other coworkers. Incredibly unprofessional, it is embarrassing for everyone involved when they don’t use the office for what an office is supposed to be for. Speaking to staff in a manner that is a little more respectful would be nice."
"I can not say enough about Stacy, the admissions coordinator there. She was so informative and helpful, and walked me through the process of being admitted to the Huntington's unit."
"The staff is very friendly and compassionate"
"very very horrible place, just sad the abuse that happens here."
